两种代谢综合征诊断标准在高血压人群中的比较

目的 比较美国国家胆固醇教育计划成人组第3次报告(NECP/ATPⅢ)以及中华医学会糖尿病学分会(CDS)代谢综合征(MS)诊断标准在高血压人群中的应用.方法 收集7 485例高血压患者人体测量学及生化检查资料,比较分别应用NECP/ATPⅢ和CDS标准,MS的检出率及诊断的一致性,并分析不同性别、年龄MS检出率及代谢异常构成的分布特点.结果 NECP/ATPⅢ和CDS标准MS检出率分别为39.4％ (n =2 949)和42.5％(n=3 181),CDS标准诊断的MS检出率较高(x2 =454.19,P＜0.001).与CDS标准相反,NECP/ATPⅢ标准女性MS检出率高于男性x2=365.58,P＜0.001);2种标准各年龄组MS检出率变化趋势相似,随着年龄增高而增高(x2=425.51,P＜0.05).结论 与NECP/ATPⅢ标准相比,CDS标准在高血压人群中MS的检出率相对较高.

Comparison of the two diagnostic criteria of metabolic syndrome among hypertensive population

Objective To compare the application of National Cholesterol Education Program's Adult Treatment Panel Ⅲ (NCEP/ATP Ⅲ) and Chinese Diabetes Society (CDS) diagnostic criteria of MS in hypertensive population.Methods Anthropometry and blood pressure were measured and bio-specimens were collected in 7 485 hypertensive patients.The detection rate of MS using NCEP/ATP Ⅲ criteria and CDS criteria in patients with hypertension were compared and the accordance of the two diagnostic criteria were studied.Characteristics of distribution of detection rate in different sex,age and metabolic abnormity were analyzed.Results The overall detection rate of MS was 39.4％ (n=2949) and 42.5％ (n =3 181) according to NCEP/ATP Ⅲ and CDS diagnostic criteria,respectively.The detection rate of CDS criteria was higher (x2 =454.19,P ＜ 0.001).The detection rate of MS was higher in males than in females using CDS criteria but this sex difference was reversed when using the NCEP/ATP Ⅲ criteria (x2=365.58,P ＜ 0.001).The changing trend with aging were no different between the two criteria,the prevalence increased with age (x2 =425.51,P ＜ 0.05).Conclusion Compared with the NCEP/ATP Ⅲ criteria,the detection rate of MS by the CDS criteria is higher in hypertensive patients.
