胃癌患者外周血淋巴细胞免疫功能的变化及临床意义

目的 检测胃癌患者外周血淋巴细胞免疫功能的变化，探讨其与胃癌新临床病理分期（TNM）及组织病理学分级的关系。方法流式细胞仪测定胃癌患者及正常对照组外周血淋巴细胞亚群及NK细胞数量。结果胃癌患者外周血CD3^＋、CD4^＋、NK、CD19^＋细胞数量及CD4^＋／CD8^＋比值较正常对照组明显下降（P〈0．01），而CD8^＋细胞水平显著升高（P〈0．01）。Ⅲ～Ⅳ期患者的CD3^＋细胞数、CD4^＋／CD8^＋比值、NK细胞数降低（P〈0．05）。胃癌组织的分化程度与细胞免疫功能无明显相关性。结论胃癌患者外周血淋巴细胞免疫功能低下，且临床病理分期越晚，其免疫功能越低。检测淋巴细胞亚群、NK细胞可用于胃癌患者的免疫状态监测。

Detection of Lymphocyte Immune Function in Peripheral Blood of the Patients with Gastric Cancer

Objective To analyze the relationship of peripheral blood lymphocytes immune function of the patients with gastric cancer to the new clinical pathological stage （TNM） and the histological types of the tumors. Methods Flow cytometry was used to detect the marks of lymphocytes subsets （CD3^＋, CD3 ^＋ CD4 ^＋, CD3^ ＋ CD8^＋, CD19^＋） and the NK cells （CD3 - CD16 ^＋ 56^ ＋） cells in blood samples from patients with gastric cancer and health people. Results Lower CD3^＋, CD4^＋ T cells, NK cells, CD19 ^＋ cells and CD4 ^＋ / CD8^ ratio were detected in blood samples from cancer group than that from health people （P〈 0.01）. CD8^＋ T lymphocytes were reverse. This change was related to the TNM pathological stage, not to the histological types of the tumors. Conclusions The immune function of lymphocytes in peripheral blood of the patients with gastric cancer was low, and even worse in the lower grade of TNM stage. It is of active significance by detecting T lymphocyte subgroup in peripheral vein in patients with cancer for estimating the function of cell immune and diagnosis.
