肺炎性假瘤的影像学表现及诊断

目的:探讨肺炎性假瘤的影像学表现和诊断。方法:回顾性分析48例确诊为肺炎性假瘤患者的X线片及CT扫描的影像学特征。结果:病变位于肺右上叶8例,肺右中叶14例,肺右下叶6例,肺左上叶12例,肺左下叶8例。X线片表现:肿块为圆形或椭圆形阴影,直径2～8 cm,边缘清晰,有"毛刺"、条索状阴影、分叶状、空洞、钙化灶等征像。CT扫描示:病灶形态表现为圆形或类圆形、直径2～8 cm;密度不均匀34例,其余密度均匀;有分叶、粗毛刺、刀切征、平直征、桃尖征等征像,内侧缘有引流支气管影10例,肿块内小空泡或支气管影8例,胸膜粘连增厚8例。X线片误诊36例(75%),CT扫描误诊12例(25%)。结论:全面综合分析肺部炎性假瘤的影像学特征,充分结合临床病史和体征可提高本病诊断准确率。

The imaging findings and diagnosis of pulmonary inflammatory pseudotumor

Objective To investigate the imaging findings and diagnosis of pulmonary inflammatory pseudotumor.Methods The retrospective analysis of 48 cases of patients admitted in our hospital was developed in our research.These patients were diagnosed for pulmonary inflammatory pseudotumor according to the imaging characteristics of X-ray and CT scanning.Results There were eight cases of lesions in the lung upper lobe,forteen cases in the lung right middle lobe,six cases in pulmonary lower lobe,twelve cases in the left upper lobe of the lung and eight cases the left lower lobe.X-ray findings: the masses were circular or oval-shaped shadow,the diameters were between 2cm and 8cm,and they had the signs of clear edge,burr,cord-like shadows,lobulated,cavity and calcificational focuses.CT scanning findings: the lesion morphology performance was round or oval,the diameters were between 2cm and 8cm;34 cases had heterogeneous density,while the rest of the cases had uniform density;they had score leaves,coarse wool needlings,jackknife,and straight and peach tip signs.There were 10 cases with bronchial shadows of medial edge,8 cases with small lumps vacuoles or bronchial shadows in the bump,and 8 cases with pleural thickening.Moreover,36 cases(75%) were misdiagnosed by X-ray,and 12 cases(25%) were misdiagnosed by CT.Conclusion Comprehensive and integrated analysis of the imaging characteristics of the pulmonary pseudotumor,fully combineing the clinical history and the signs would improve the diagnostic accuracy of this disease.
