老年慢性阻塞性肺疾病患者T淋巴细胞亚群的临床观察

目的：探讨T淋巴细胞亚群中CD分子在慢性阻塞性肺疾病(COPD)发病中的作用及与病程的关系。方法：随机收集住院老年COPD患者急性发作期和缓解期空腹静脉血43份，健康老年人12份、非老年人20份，分别用流式细胞仪测定DNA含量。结果：健康老年组CD3^ 、CD4^ 明显下降，与非老年组比较有显著性差异(P＜O．05)。老年COPD组急性发作期CD3^ 、CD4^ 与健康老年组、非老年组、老年COPD组缓解期比较均有显著性差异(P＜O．01)，CD8^ 在各组问均无显著差异性。CD4^ ／CD8^ 随患者年龄增大，COPD病情加重，其比值呈下降趋势。结论：老年COPD患者T淋巴细胞亚群CD3^ 、CD4^ 下降以及CD4^ /CD8^ 比例失调，尤其是急性发作期更为显著，提示老年COPD患者的T淋巴细胞免疫功能低下，因此，老年COPD急性发作期在综合治疗的同时提高患者免疫功能十分重要。

T lymphocyte subpopulation in senile patients with chronic obstruction pulmonary disease

Objective:To study the function of CD molecules in T lymphocyte subpo pulation during the attack of COPD and the relationship between CD molecules and the course of the disease. Methods: Fasting venous b lood samples were collected in three groups:senile patients with COPD at the acute attack stage or remissi on stage (n=43), healthy senile group (n=12), non-senile group (n=20 ). The DNA c ontents were detected. Results: CD3+ and CD4+ in healthy senile group were lower t han that in non- senile group (P<0.05). Compared with healthy senile gro up, non-senile group and senile COPD group at the remission stage, the contents of CD3+ and CD4+ in senile COPD group at the attack stage showed significant differen ce (P<0.01). As for CD8+, no significant difference was found in three gro ups. CD4+/C D8+ was increased with the growth of age and decreased with exacerbation of CO PD. Conclusion:The decrease of CD3+ and CD4+ and the imbalance of CD4+/CD8+, especially at the stage of acute attack, prompt the hypo-immunity of T lymphocyte in senile p atients of COPD. It is important to enhance the immunologic function during the treatment of senile patients of COPD at the stage of acute attack.
