骨质疏松性椎体压缩骨折行单双侧椎体后凸成形术的对比研究

目的评价单、双侧经皮椎体后凸成形术（percutaneous kyphoplasty，PKP）治疗腰椎骨质疏松性椎体压缩性骨折（nsteoporotic vertebral complression fractures，OVCF）的差异。方法回顾性分析2008年7月-2010年2月I27例经单侧或双侧椎弓根行PKP治疗的OVCF患者，采用疼痛视觉模拟量表（visual analog scale，VAS）评分、活动能力评分、Cohb角度及椎体高度恢复率比较分析。结果术所有患者疼痛均缓解，随访〉12个月。2组手术前后VAS评分、活功能力评分和Cobb角的差异均有统计学意义（P〈0．05）。2组手术前后椎体前缘、中柱高度差异有统计学意义（P〈0．05）．但后缘高度变化差异无统计学意义（P〉0．05）。末次随访时，椎体前缘、中柱和后缘高度与术后相比较差异无统汁学意义（P〉0．05）；2组问比较差异均无统计学意义（P〉0．05）。结论单双侧PKP治疗OVCF均可取得满息的治疗效果，而单侧PKP手术时间更短、骨水泥渗漏率更低。

Comparison of percutaneous kyphoplasty between unilateral and bilateral approach in treatment of osteoporotic vertebral compression fractures

Objective To evaluate the difference of curative effects of percutaneous kyphoplasty （PKP） between unilateral and lfilaleral for osteoporotic vellehral compression fractures （OVCF）. Methods A total of 127 patients with OVCF operated bv PKP fi＇om July 2008 to Febrnarv 2010 were retrospectively analysed, and they w-ere divided into 2 groups on the basis of unilateral and hilateral PKP. Visual analog scale （VAS） score, locomotor activity, Cobb＇ s angle and recovery rate of vertebral body height were corresponding recorded and analyzed. Results All patients received more than 1 year follow-up and alleviated pain well postoperalively. Between pre- and post-operation, the differences of the mean visual analogue scales, locomolor aetivitv and Cobb＇ s angle were statistically significant in 2 groups （ P 〈 0.05 ）. The differences of height of anterior border and center pillar between pre- and post-operation were statistically significant in 2 groups （P 〈0.05） , but the difference of heigh! of posterior border weren＇t statistically significant（ P 〉0.05）. Compared with post-operation, the differences of the above-meutinned imlex weren＇t statistically significant at the final follow-up（ P 〉 0.05 ）. The differences of the curative effects weren＇ I statislically significant between 2 groups（ P 〉 0.05 ）. Conclusion corresponding in above-mentioned index, there were no slatisfieal significance differences. Conclusion Both unilateral and bilateral PKP in treatment of OVCF can obtain satisfactory therapeutic effeets; but in the treatment by unilateral PKP, the operation time is shorter and bone cement leakage rate is lower.
