锥形束CT在诊断上颌阻生尖牙邻近切牙根吸收中的应用价值

摘    要:目的 对比研究锥形束CT与曲面体层片在诊断上颌阻生尖牙邻近切牙根吸收的差异.方法 对46例上颌阻生尖牙患者拍摄曲面体层片和锥形束CT片,比较这两种方法对阻生尖牙邻近切牙根吸收程度的诊断率.结果 曲面体层片对上颌阻生尖牙邻近切牙根吸收诊断的灵敏度为37.5%,特异度为94.1%,误诊率为5.9%,漏诊率为62.5%,正确度为76.0%.曲面体层片和锥形束CT片在诊断切牙根吸收上的一致性差(Kappa值=0.364).结论 锥形束CT能清晰显示上颌阻生尖牙邻近切牙的根吸收程度,曲面体层片对上颌阻生尖牙邻近切牙根吸收的诊断存在漏诊,对存在上颌阻生尖牙的病例,建议应用锥形束CT来辅助诊断,以避免漏诊.

Abstract:Objective To compare the difference of cone beam computed tomography (CBCT) and panoramic tomography in the diagnosis of the resorption of neighboring incisors with impacted maxillary canines.Methods 46 patients with impacted maxillary canines were subjected to both CBCT and panoramic tomography.The accuracy of diagnosis by the two examinations in the extent with the root resorption of neighboring incisors was observed and compared.Results The sensitivity,specificity,misdiagnosis rate and rate of missed diagnosis of diagnosing the incisors root resorption caused by impacted maxillary canines using PTG were 37.5% and 94.1% and 5.9% and 62.5% respectively.There was a low consistency(k =0.364) in diagnosing the incisors root resorption using PTG and CBCT.Conclusion The root resorption extent of neighboring incisors with impacted maxillary canines was clearly observed by CBCT.Panoramic tomography may misdiagnose the resorption of neighboring incisors with impacted maxillary canines.CBCT is more accurate in the diagnosis of impacted maxillary canines and helpful in making treatment plans for the doctors.
