激素冲击疗法与常规疗法治疗儿童肾病综合征的临床疗效对比分析

目的:为儿童肾病综合征合理选用激素给药方式提供依据。方法:回顾分析65例儿童肾病综合征的病例资料,其中甲基强的松龙冲击疗法组33例,泼尼松常规疗法32例,分别给予甲基强的松龙冲击疗法和泼尼松常规治疗诱导缓解,在两组患儿蛋白转阴后,均继续予泼尼松维持及减量常规治疗1～2年后停药,比较两组临床疗效、不良反应发生率差异。结果:甲基强的松龙冲击疗法组的总有效率为93.94%,泼尼松常规疗法组的总有效率为75%,两组差异具有统计学意义(P<0.05)。结论:甲强龙冲击治疗儿童肾病综合征可稳定缓解症状,提高完全缓解率,短期使用未增加不良反应,却能减轻中长期治疗的不良反应,建议初次使用的患儿,可用足量激素治疗12周,总疗程应至少大于1年。

The analysis of comparation between the impact of hormone therapy with conventional therapy to treat children with nephrotic syndrome

Objective: To Provide a reference for treating children with nephrotic syndrome.Methods:To make a retrospective analysis of 65 children with nephrotic syndrome,including methylprednisolone pulse therapy group 33 cases,conventional therapy of prednisone 32 cases.They were treated with methylprednisolone pulse therapy and prednisone conventional treatment as remission induction respectively.After protein was negative,continue to maintain and reduct prednisone for 1 to 2 years,and compare the efficacy and adverse reactions.Results:The total effective rate of Methylprednisolone pulse therapy group was 93.94%,the difference was statistically significant between two group(P<0.05).Conclusion:The methylprednisolone pulse therapy can be stable to relieve symptoms,improve the complete remission rate,short-term use did not increase adverse effects,but can reduce the long-term treatment of adverse reactions in the treatment of children with nephrotic syndrome.It is recommended that the children who are applied with this method can be treated with enough hormone for 12 weeks,and the total course of treatment should be at least greater than 1 year.
