术前区域动脉化疗治疗进展期结直肠癌的临床价值

目的评价术前区域动脉灌注化疗在进展期结直肠癌的临床效果。方法60例进展期结直肠癌分两组，PRAC组30例行术前区域动脉灌注化疗（PRAC）、介入后1～2wk手术切除，对照组30例直接接受手术治疗。结果PRAC能够明显改变结直肠癌组织的病理组织形态，总有效率95.83％。PRAC组与对照组的根治性切除率比较为86.66％（26／30）：73．33％（22／30）；术后1年生存率96．66％：83.33％；术后3年生存率86．67％：66．67％；术后3年复发率13．33％：26．67％；〈7cm低位直肠癌的保肛率75％：53．33％。结论PRAC能抑制或杀伤了大量敏感的肿瘤细胞，使肿瘤缩小，有利于手术切除从而提高进展期结直肠癌的根治性切除率及术后生存率、减少复发率，提高低位直肠癌的保肛率，是一种有效的新辅助治疗措施。

The value of preoperative regional intra-arterial chemotherapy in treatment of progressive colorectal cancer

Objective To evaluate the clinical results of progressive colorectal cancer treated by preoperative regional intra_arterail chemotherapy（PRAC）. Methods There 60 patients were divided into PRAC group consisted of 30 treated with PRAC and the cancer were oiperationally removed one or two weeks after PRAC and control group also consisted of 30 cases treated with operationd. Results The total effective rate in PRAC group was 95.53%. The rates of radical excision in PRAC group and control grounp were 86.66% and 73.33% respectively, The - year, 3 - year survival rates and reccurrent rate 3 years after treatment in the PRAC group were 96.66%, 86.67% and 13.33%, while that of the control group were ： 83. 33%, 66.67% and 26.67%. The rates of protective annus of lower rectal cancer （ 〈 7cm） in the PRAC and control groups were 75% and 53.33%, respectively. Conclusion PRAC could depress or kill most of the sensitive tumour cells helpful for surgical treatment and enhance the survival rate, thus it is a new supplemented therapy for colorectal cancer.
