白内障囊外摘除术中后囊破裂原因分析

本文对作者施行现代囊外白内障摘除术中发现的106例后囊破裂的原因，临床特征及水中处理进行了分析．其中外伤性白内障的22例术前已存在后囊的裂孔，多是由穿通性眼外伤所致．裂孔的边缘较厚，在晶体皮质抽吸过程中无扩大和玻璃体的脱出，并顺利植入了后房人工晶体．另外84例为老年性白内障囊外术中新鲜形成的后囊破裂，其边缘较薄，术中有扩大的趋势，其中47例（56％）经处理后植入了后房人工晶体．术中后囊破裂的主要原因是机械性损伤．本文对后囊破裂的术中处理，粘弹性物质及玻璃体切割在术中的应用进行了讨论．

Etiological Analysis and Treatment of the Lens Posterior Capsular Breaks During the ECCE

The pathological and clinical featues and the surgical management of lens posterior capsular breaks in 106 patients during the extracapsular cataract extraction (ECCE) were studied etiologically. Posterior capsule breaks in the 22 cases of traumatic cataract were preexisting with the thick fibrosed margins and remained same size during the irriga tion/ aspiration.The posterior chamber intraocular lenses were implanted in all these cases. In the senile cataract group (84) cases, the posterior capsule breaks were fresh with thin margin and tended to enlarge during irrigation/aspiration.47(56%) of these cases were implanted the posterior chamber intraocular lenses succesfully. It is considered that properly deal with the posterior capsule breaks using the viscoelastic material, anterior vitrectomy and preserve afficient posterior capsule, the posterior chamber intraocular lens can be implanted with less complications.
