呼吸道分泌物铜绿假单胞菌的耐药性研究

摘    要:目的了解呼吸道感染铜绿假单胞菌PA的耐药性变化,指导临床合理使用抗生素。方法用autoscan-4细菌鉴定仪对166株PA进行菌种鉴定、药敏试验及诱导型β-内酰胺酶(IB)检测,统计耐药率,做统计学分析。结果PA产酶率为89.2%;哌拉西林/三唑巴坦、头孢他啶、阿米卡星、环丙沙星、亚胺培南、安曲南、哌拉西林、头孢曲松和庆大霉素等抗生素耐药率较低,为15.1%~45.8%。产IB株对阿米卡星、安曲南、头孢唑肟和头孢曲松等的耐药性与不产IB株相比无显著性差异(P>0.05),其余均有显著性差异(P<0.01)。结论PA产IB株呼吸道感染严重,因此加强耐药性监测,根据分离株的耐药特点合理用药非常重要。

Abstract:Objective To study the antibiotic resistance change of Pseudomon aseruginosa(PA) in respiratory tract secrete. Methods Strain identification, drug sensitivity testing and inducer-β-lactamase(IB) were determined with AS-4 and NC21 for 166 strains of PA. The data of tests were analysed statistically. Results The production rate of IB was 89.2%. The resistance rate of Piperacillin/tazobactam, Ceftazidime, Amikacin, Ciprofloxacin, Imipenem, Aztreonam, Piperacillin,ceftriaxone and Gentamicin were 15.1%-45.8%.The resistance rate of PA producing IB compared with no producing IB,beside Amikacin, Aztreonam,Ceftizoxime and ceftriaxone,the other were salience difference.Conclusions Infection in respiratory tract is severity with producing IB strains. It is very important to select antibiotics correctly for infections in respiratory tract according to the results of drug susceptibility tests.
