食管癌和癌旁组织C-erbB-2的表达研究

摘    要:目的:分析癌组织和癌旁组织C-erbB-2的表达状况,探讨食管癌变过程中细胞增生调控的分子机制。方法:66例原发性食管癌手术切除标本,癌旁组织作"席卷样"包埋,并记录切片中孤立的正常上皮及各种病变数目,即基底细胞过度增生(BCH)、不典型增生(DYS)和鳞癌(SCC),并利用免疫组织化学(ABC)法检测这些组织中c-erbB-2表达的变化。结果:从正常食管上皮到BCH、DYS、SCC,C-erbB-2阳性表达率分别为14%、60%、37%和89%,C-erbB-2局灶型分布的百分率分别为50%、67%、48%和35%,弥漫型分布的百分率分别为50%、33%、52%和65%;从高分化到中分化、低分化SCC时C-erbB-2阳性表达率分别为94%、100%和75%。结论:C-erbB-2表达变化发生在食管癌变的早期阶段,C-erbB-2表达阳性率从正常上皮到BCH升高明显,到癌时最高,提示:C-erbB-2可能是食管癌变发生的主要分子事件之一;从正常食管上皮到BCH、DYS、SCC,C-erbB-2局灶型分布百分率逐渐减少,弥漫型分布百分率逐渐增加,提示:随着癌变的进展,C-erbB-2表达量逐渐增加。

Abstract:Objective:To analyze the expression of C-erbB-2 in esophageal cancer tissues and adjacent cancer tissues,and approach the molecular mechanism in the process of esophageal carceration.Methods:To detect the variation of expression of C-erbB-2 by immunohistochemistry(ABC) in 66 cases of primarily esophageal cancer and matched adjacent cancer samples by exairesis,all samples were reel-like embedded,and record the number of isolated normal epithelial tissue and various kinds of lesions,i.e.,basal cell hyperplasia(BCH),dysplasia(DYS) and squamous cell carcinoma(SCC).Results:The positive rates of C-erbB-2 in normal esophageal epithelium,BCH,DYS,SCC were 14%,60%,37% and 89%,respectively,meanwhile,From normal esophageal epithelium to BCH,DYS,SCC,the positive rates of C-erbB-2 in the focal pattern and in the diffuse pattern of all tissues above were 50%,67%,48%,35% and 50%,33%,52%,65%,respectively.The positive rates of the C-erbB-2 in well differentiated,moderately differentiated,poorly differentiated SCC were 94%,100% and 75%,respectively.Conclusion:The alterations of C-erbB-2 protein expression occurred in early stage of esophageal carcinogenesis and the positive rates obviously increased with the lesions progressed,suggested that C-erbB-2 may be one of important molecular events involved in esophageal carcinogenesis.From normal esophageal epithelium to BCH,DYS,and SCC,the positive rate of C-erbB-2 in focal pattern of all tissues above decreased and that in diffuse pattern increased,suggested the expression of C-erbB-2 increased accompany with esophageal carcinogenesis.
