乳腺癌患者中乳腺球蛋白的表达及其临床意义

目的 探讨hMAM mRNA在乳腺癌患者皮肤组织及乳头乳晕复合体的表达及其临床意义.方法 对30例乳腺癌患者,术后皮肤在距肿瘤最近点取材,乳头乳晕复合体经正中切面取材,同时取乳腺癌组织26例,正常皮肤组织10例,采用RT-PCR技术检测hMAM mRNA.结果 乳腺癌组织均阳性,正常皮肤组织均阴性.乳腺癌患者皮肤组织hMAM mRNA阳性率为10.0%(3/30),乳头乳晕复合体hMAM mRNA阳性率为16.7%(5/30).皮肤组织hMAM mRNA阳性率与皮肤外观是否有改变有关(P=0.0035).乳头乳晕复合体hMAM mRNA阳性率与肿瘤是否位于中心区有关(P=0.0192).结论 乳腺癌患者hMAM mRNA阳性预示着微转移.皮肤局部外观的改变,及肿块的相对位置能够大致预测皮肤或乳头是否受累;而且,由于皮肤及乳头的受累率较低,保留皮肤的乳腺癌根治术,或腔镜乳腺癌根治术具有可行性.

The clinical significance of hMAM mRNA expression in the local skin of breast cancer

Objective To define the expression of hMAM mRNA in the local skin and the nipple-areola complex(NAC) of breast cancer and its clinical significance.Methods Samples from both the regional skin nearest to the lump and the midline-transection of the nipple-areola complex(NAC) were collected from thirty randomized patients with breast cancer and examined for hMAM mRNA expression by RT-PCR technique.The expression of hMAM mRNA in 26 samples of breast cancer tissue and 10 samples of normal skin tissue were also examined as control.Results The samples from cancerous tissue were all positive,while those from normalskin negative.The positive rate of hMAM mRNA in the local skin and in the NAC of breast cancer was 10.0%(3/30)and 16.7%(5/30),respectively.The hMAM mRNA positiveness of the local skin and the NAC was related to the presence of visual alterations(P=0.0035) and to the central orientation of the lump(P=0.0192) respectively.Conclusion The positiveness of hMAM mRNA indicates micro-metastasis.Skin alterations and lump orientation can help predict nipple-skin invasion by breast cancer.Moreover,as the incidence of nipple-skin invasion is relatively low,the application of skin-sparing or laparoscopic techniques is practically feasible.
