| 摘 要： | 目的观察动力型髓内钉治疗肱骨三段骨折、粉碎性骨折的手术治疗效果,分析影响疗效因素。方法自2005年3月～2008年3月收治17例肱骨粉碎性骨折(粉碎组)及8例肱骨三段骨折(三段组),采用顺行髓内钉固定技术动力型固定,术后定期随访摄X线片,观察骨折愈合情况,并采用Costant评分系统对肩关节功能评分,分析影响疗效的因素。结果所有患者接受1～3年的随访,粉碎组与三段组随访时间无明显差异。粉碎骨折术后愈合时间(5.5±2.4)个月,三段骨折愈合时间(8.0±3.2)个月,两者差异有统计学意义。末次随访肩关节Costant评分粉碎组(95.0±6.5)分,三段组(94.6±7.0)分,两者差异无统计学意义。结论根据骨折位置、类型等情况选择动力型髓内钉治疗肱骨粉碎性骨折和三段骨折,可取得良好的治疗效果。术中应注意进钉位置的正确选择,包埋髓内钉近端,修补肩袖组织,肩关节功能不会受到明显影响。
