BiPAP通气治疗重症哮喘合并呼吸衰竭的临床分析

目的探讨双水平气道正压（BiPAP）通气在重症哮喘合并呼吸衰竭的患者治疗中的意义。方法回顾性分析2004年4月至2009年6月住院重症哮喘合并呼吸衰竭患者39例,治疗组23例,在常规治疗基础上使用BiPAP通气治疗;对照组16例,使用常规治疗。监测治疗组通气治疗前后临床表现及呼吸、心率、平均动脉压、动脉血气分析等指标的变化,并与对照组作比较。结果治疗组与对照组治疗后各参数（除pH外）比较差异有显著性意义（P〈0.05）,临床症状明显改善,死亡率明显降低,平均住院时间缩短。结论在常规治疗的基础上早期应用BiPAP呼吸机治疗重症哮喘合并呼吸衰竭,是一种安全有效的治疗方法。

Clinical Analysis of BiPAP Ventilation for Severe Asthma Combined with Respiratory Failure

Objective To discuss the values of face-masked double horizontal positive airway pressure（BiPAP）ventilation in the treatment of severe asthma merge respiratory failure.Methods Clinical data of 39 cases of severe asthma merge respiratory failure were retrospectively analyzed who were treated in our hospital from April 2004 to June 2009.In the treatment group,23 patients were given BiPAP treatment besides routine remedies.In the control group,16 patients were only given the same routine remedies as those in the treatment group.The RR,HR,MAP,and arterial blood-gas were monitored and compared between the two groups.Results All the parameters except pH showed significant difference between the two groups （P〈0.05）.Clinical signs were remarkably ameiorated and the death rate decreased obviously,the average time stay in hospital shortened in the treatment group.Conclusion Early application of BiPAP on the basis of conventional remedies is a safety and effective method for severe asthma combined with respiratory failure patients.
