脑积水手术效果及脑室-腹腔分流管的选择探讨

目的探讨脑室-腹腔分流术治疗脑积水的分流管的选择及提高分流手术疗效的途径。方法回顾性分析2007年9月至2010年3月脑室-腹腔分流术治疗52例脑积水患者的临床资料。52例患者采用高压管分流4例,采用中压管分流25例,采用低压管分流19例,采用可调压管分流4例;其中更换分流管4例。结果术后一周临床症状缓解33例(63.5%,33/52),出院时症状缓解41例(78.8%,41/52)。43例随访3～30个月,脑积水症状改善23例,无显著变化3例,7例症状较术前加重。结论脑室-腹腔分流术仍是治疗脑积水的首选方式;对个体化病人应选择最适合分流压力的分流管进行分流以达到最佳手术效果,如何选择颅内压相适宜的分流装置目前仍需进一步探讨。

Curative effect of ventriaculoparitoneal shunt on hydrocephalus and choice of ventriculoperitoneal shunt valve
