内镜下三线减张鼻中隔矫正术的临床效果及患者心理健康状况

目的分析内镜下三线减张鼻中隔矫正术的疗效以及对患者心理健康状况的影响。方法对2009年10月—2013年2月在成都市第五人民医院经鼻内镜结合鼻窦CT检查确诊为鼻中隔偏曲的138例患者行三线减张鼻中隔矫正术,对患者治疗效果进行评价,采用SCL-90量表对患者手术前后的心理健康状况进行调查分析。结果 138例患者中,治愈125例（90.6%）、好转13例（9.4%）;鼻中隔偏曲患者手术前在躯体化、人际关系敏感、抑郁、焦虑、偏执、精神病性6个方面的因子评分高于手术后评分,差异有统计学意义（P〈0.05）;手术前检出例数最多的前5位健康因子依次为：躯体化（11.6%）、精神病性（10.1%）、人际关系敏感（8.0%）、焦虑（7.2%）和抑郁（6.5%）,手术后患者心理健康因子检出率均低于5%,手术前后心理健康因子检出率比较发现,躯体化和精神病性改变最为明显,差异有统计学意义（P〈0.05）。结论鼻内镜下三线减张鼻中隔矫正术处理鼻中隔偏曲疗效确切,患者心理健康状况与临床疗效改善有很大关系。

Clinical effect and mental health status in patients of endoscopic surgery for deviation of nasal septum with three high tension lines resection

Objective]To analyze the clinical effect and mental health status of patients of endoscopic surgery for deviation of nasal septum with three high tension lines resection. Methods]138 patients diagnosed deviation of nasal septum by endoscopic sinus and CT in our hospital from September 2009 to March 2013 were carried out a three high tension lines resection surgery,the therapeutic effects were evaluated,the mental health status of patients before and after surgery were analyzed by SCL-90 Scale. Results]Among 138 patients,125 cases were cured（ 90.6%）,13 cases were improved（ 9. 4%）. The score of somatization, interpersonal sensitivity,depression,anxiety,paranoia and psychoticism before surgery were higher than that after surgery,and the difference was statistically significant（ P〈0. 05）. The top five health factors detected before surgery were somatization（ 11. 6%）, psychoticism（ 10. 1%）,interpersonal sensitive（ 8. 0%）,anxiety（ 7. 2%） and depression（ 6. 5%）,the detection rate of mental health factors after surgery was less than 5%. Compared the detection rate of mental health factors before and after surgery,the change of somatization and psychoticism was the most obvious and the difference was statistically significant（ P〈0. 05）. Conclusion]The efficacy of endoscopic surgery for deviation of nasal septum with three high tension lines resection is exact. The mental health status of patients has a relationship with improving the clinical efficacy.
