苏州市相城区某镇2008年与2012年居民死因和减寿年比较

目的了解2008与2012年苏州市相城区某镇居民死亡率、死亡原因、减寿情况变化,为制定疾病防控工作重点提供科学依据。方法利用相城区某镇2008—2012年居民死因监测数据,按照国际疾病分类ICD-10标准进行统一编码,使用Excel 2003进行数据整理和统计分析,计算死亡率、潜在减寿年数（PYLL）及潜在减寿率（PYLL‰）等指标。结果 2008年该镇居民粗死亡率为708.02/10万,前5位死因依次为循环系统疾病、恶性肿瘤、呼吸系统疾病、损伤和精神行为障碍,占全部死因的87.60%。全死因减寿年数为3 445人年,减寿率为70.29‰,平均减寿数为9.93岁。2012年该镇居民粗死亡率为755.07/10万,前5位死因依次为恶性肿瘤、循环系统疾病、损伤、呼吸系统疾病和症体临床异常,占全部死因的79.67%。全死因减寿年数为3 036人年,减寿率为63.86‰,平均减寿数为8.46岁。结论恶性肿瘤、循环系统疾病、呼吸系统疾病等慢性非传染性疾病和损伤仍然是该镇居民生命健康的主要威胁,其中肿瘤和损伤的PYLL分别位居2008和2012年PYLL的第1位,应加强对这几类疾病尤其是肿瘤和损伤的综合防治。

Comparison on the causes of death and the potential years of life lost of residents in a town of Xiangcheng district in Suzhou City in 2008 and 2012

Objective ] To understand the death rate, main causes of death and the potential years of life lost （PYLL） among resi- dents in a town of Xiangcheng District in Suzhou City in 2008 and 2012, provide scientific basis for formulating the emphasis of disease control and prevention work. Methods ] The monitoring data of death cause of residents in a town of Xiangcheng district from 2008-2012 were encoded by ICD-10 classification and analyzed by Excel 2003. The death rate, potential years of life lost （ PYLL） and the rate of potential years of life lost （ PYLL‰ } were calculated. Results ] The crude death rate of the residents in the town was 708.02/100 000 in 2008. The top five causes of death were circulation system disease, malignant tumors, respiratory disease, injuries and mental and behavioral disorders which accounted for 87.60% of all causes of death. The PYLL of all causes of death were 3 445, the rate of PYLL was 70.29‰,and the average years of life lost （AYLL） was 9.93. The crude death rate of the residents in the town was 755.07/100 000 in 2012. The top five causes of death were malignant tumors, circulation system disease, injuries, respiratory disease and disease clinical abnormalities which accounted for 79.67% of all causes of death. The PYLL of all causes of death were 3 036, the rate of PYLL was 63.86%%and the average years of life lost was 8.46. Conclusion] Chronic noninfectious diseases like malignant tumors, circulation system disease and respiratory disease and injuries are the main health threats of residents in the town. The PYLL of tumors and injuries are the top one of PYLL in 2008 and 2012 respectively. We should strengthen the comprehensive control and prevention of these kinds of diseases especially tumor and injury.
