奥氮平和利培酮对精神分裂症患者血糖和血脂的影响

目的探讨奥氮平和利培酮对精神分裂症患者体重、血糖和血脂的影响,对比其差异。方法选取60名精神分裂症患者,随机分为奥氮平组和利培酮组各30例,分别给以奥氮平和利培酮治疗,于入组时和28 d后分别测量体重、血糖和血脂水平,同时用阳性与阴性症状量表（PANSS）评定精神症状。结果与治疗前相比,2组患者体重均显著增加,差异有统计学意义（P〈0.01）,总胆固醇（TCH）、甘油三酯（TG）和低密度脂蛋白（LDL）水平明显升高,差异有统计学意义（P〈0.05）,血糖和高密度脂蛋白（HDL）水平变化不明显,差异无统计学意义（P〉0.05）;治疗前后2组患者体重、血糖、血脂指标相比差异均无统计学意义（P〉0.05）。治疗后奥氮平组PANSS量表分值明显低于利培酮组（37.90±9.884）vs（43.66±9.37）],差异有统计学意义（P〈0.05）。结论奥氮平和利培酮对精神分裂症患者体重、血脂均有明显影响,奥氮平降低PANSS分的作用大于利培酮。

Effect of olanzapine and risperidone on blood glucose and blood lipids of patients with schizophrenia

Objective ] To explore the effect of olanzapine and risperidone on body weight, blood glucose and blood lipids of patients with schizophrenia , compare the differences. Methods] 60 patients with schizophrenia were randomly divided into the olanzapine group and the risperidone group, 30 cases in each group. Two groups were given olanzapine and risperidone respectively. The levels of body weight, blood glucose and blood lipids were measured at baseline and after 28 days treatment, and the Positive and Negative Syndrome Scale （PANSS） was applied to assess the psychological symptoms. Results] The values of body weight in both groups after treatment were higher than those before treatment, and the differences were significant （ P 〈0.01 ）. The levels of total cholesterol （ TCH ）, triglyceride （ TG） and low density lipoprotein （ LDL ） increased significantly （ P 〈 0.05 ）. The differences in the levels of blood glucose and high density lipoprotein { HDL ） were not significant （ P 〉 0.05 ）. There were no significant differences in levels of body weight, blood glucose and blood lipids before and after treatment between two groups （ P 〉 0.05 ）. After treatment, scores of PANSS in olanzapine group （ 37.90± 9. 884 ） was lower than those in risperidone group （ 43.66 ± 9.37 ） , and the differ- ence was significant （P 〈 0.05 ）. Conclusion] Olanzapine and risperidone have significant effect on body weight and blood lipids in patients with schizophrenia, and olanzapine shows more obvious impact on reducing PANSS scores than risperidone.
