脑溢安颗粒对急性脑出血病人血清白介素-6的影响

摘    要:目的:为探讨脑溢安颗粒治疗急性脑出血的疗效机制。方法:对33例急性脑出血患者采用脑溢安颗粒(脑溢安组)和西药(西药对照组)治疗,观察患者血清中白介素-6(Interleukin-6,IL-6)水平的动态变化。结果:西药对照组患者血清中IL-6含量在发病第1d,第3d,第7d均较健康人对照组明显增高(P<0.01),且在发病第1d达峰值,第14d已趋于恢复;脑溢安组患者血清IL-6含量在发病第1d,第3d与西药对照组相当,但第7d仍维持较高水平,第14d仍高于健康人对照组,与西药对照组比较有显著差异(P<0.05),患者脑出血量越大或病情越重则IL-6水平越高,尤以病程第1d最明显,结论:提示发颗粒能有效地维持急性脑子同血患者血清IL-6含量的恒定表达,在脑出血损伤中发挥神经保护作用,脑出血24h内患者血清IL-6产生水平可能预示最终的脑损伤程度,因此测定脑出血急性期血清L-6水平,可能有助于判断脑出血量和预后。

Abstract:Objective To investigate the therapeutic m echanism of Nao Yi An(NYA) granule on acute cerebral hemorrhage (ACH). Methods 33 cases of ACH were treated with NYA granule (NYA group) or with western medicine (WM group) as control. The time course and relationship o f the outcome of IL-6 in their serum were investigated. Results Significantly increased levels of the serum IL-6 in WM group (P<0.001) were observed on day 1, 3 and 7 comp ared with healthy control subjects, with a peak level on day 1, then the levels decreased successi vely and do wn to normal expressing level on day 14. The serum IL-6 contents in NYA group did not show any significant difference compared with WM group on day 1 and 3,but t h ey remained significantly higher on day 7 and day 14 than in WM group (P <0.05). The serum IL-6 contents increased remarkably accompanied by the increa se of volume of hemorrhage or the gravity of the disease especially on day 1. Conclusions It is indicated that the NYA granule may be exerting neuroprotective actions by effect ively maintaining the IL-6 expressing level in the serum of ACH. The producti on of serum IL-6 can, within the first 24 hours of the ACH onset, predict the defin it ive extent of the brain damage. Thus, determination of serum IL-6 level in th e ACH may predict the volume of cerebral hemorrhage and the prognosis of the pat ient.
