动态心电图监测在变异型心绞痛中的应用研究

目的探讨动态心电图在变异性心绞痛诊断中的价值。方法对经湖北医药学院附属东风医院确诊的70例变异型心绞痛患者进行常规12导联心电图和MarquetteHolter心电监测仪检查，比较两种检测方法中ST段、T波、室性早搏等指标的变化，并对患者行冠状动脉造影（CAG），对其检测结果进行回顾性分析。结果冠状动态造影检查示冠状动脉狭窄程度为无明显病变的10例，轻度狭窄的15例，中度狭窄的37例，重度狭窄的8例，而且阵发性ST段抬高的高度、持续时间与冠脉狭窄的程度呈正相关。同时常规心电图对变异型心绞痛的检测率明显低于动态心电图，两者相比差异显著（P〈0．05）。结论动态心电图准确完整地记录变异型心绞痛发作的全过程，对诊断变异型心绞痛具有重要的临床应用价值。

Study of application of Holter monitoring in variant angina

Objective To investigate the dynamic value of the ECG in the diagnosis of variant angina. Methods Total- ly 70 cases diagnosed in our hospital, patients with variant angina were conventional 12 -lead ECG and Marquette Hol- ter ECG monitor to check the changes Of the ST segment, T wave, ventricular premature beats and other indicators com- pare two detection methods, and patients underwent coronary angiography（CAG） of their test results were analyzed retro- spectively. Results The coronal dynamic angiography showed coronary artery stenosis of 15 cases, no obvious lesions 10 cases ,mild stenosis and moderate stenosis of 37 cases of severe stenosis, 8 cases, and paroxysmal ST segment eleva- tion height duration was positively Correlated with the extent of coronary artery stenosis. ECG detection of variant angina was significantly lower than that of＇the dynamic ECG, the difference was significant （ P 〈 0.05 ）. Conclusions The dynamic ECG accurate and complete record of the whole process has important clinical value in the diagnosis of variant angina pectoris variant arlgina peetoris.
