胰岛素强化治疗2型糖尿病合并冠心病心衰的临床观察

目的观察胰岛素强化治疗对2型糖尿病合并冠心病心衰患者的影响。方法选取焦作市第四人民医院2009年1月至2012年6月住院治疗的2型糖尿病合并冠心病心衰患者107例，随机分为治疗组、对照组，治疗组给予胰岛素泵持续皮下注入门冬胰岛素，对照组给予精蛋白锌胰岛素30R皮下注入，观察患者血糖达标时间、糖化血红蛋白、血脂、左室射血分数的变化。结果治疗后血糖达标时间治疗组（5．20±1．38）d，对照组（8．14±1．47）d；糖化血红蛋白：治疗组（7．23±1．08）％，对照组（7．87±1．23）％；左室射血分数t治疗组（57．31±4．26）％，对照组（50．28±5．12）％。两组比较差异有统计学意义（P〈0．05）。结论胰岛素强化治疗2型糖尿病合并冠心病心衰，血糖达标时间短，心功能改善明显。

Clinical observation of intensive insulin treatment of type 2 diabetes and coronary heart disease heart failure

Objective To observe the effect of intensive insulin therapy in type 2 diabetes and coronary heart disease in patients with heart failure. Methods Our hospital from January 2009 to June 2012 hospitalized type 2 diabetes and cor onary heart disease in patients with heart failure, 107 eases were randomly divided into the treatment group and the con trol group, the treatment group received insulin pump continuous subcutaneous injection of insulin aspart The control group was given a subcutaneous injection of protamine zinc insulin 30R,ohserve the patient~ blood glucose, glycosylated hemoglobin, lipids, and changes in left ventrieular ejection fraction. Results After treatment, blood glucose control treat ment group （5.20 ~ 1.38）days in the control group （8.14 ~ 1.47） days; Glycosylated hemoglobin t the treatment group （ 7. 23 ~ 1.08 ） % , the control group （ 7. 87 ~ 1.23 ） % ; Left ventricular ejection fraction t the treatmerit group （ 57, ~ i ~ 4. 26） %, control group （ 50. 28 ~ 5.12 ） %. Conclusion Intensive insulin treatment of type 2 diabetes and coronary heart disease with heart failure ,the time of blood glucose control, improved heart function.
