不同肠内营养支持时机对重型颅脑损伤术后并发感染的影响研究

摘    要:目的分析不同肠内营养支持时机对重型颅脑损伤术后并发感染的影响。方法回顾性分析河北省保定市第一医院2009年4月—2011年9月手术治疗的重型颅脑损伤患者94例,将早期肠内营养(EEN)的50例患者作为A组,延迟肠内营养(DEN)的44例患者作为B组,比较分析两组患者的术后感染发生率、感染部位和感染持续时间。结果A组的感染发生率为32.0%、持续时间为(6.8±1.5)d,B组的感染发生率为27.3%、持续时间为(7.0±1.2)d,组间差异均无统计学意义(P〉0.05),两组患者均以肺部感染多见。结论不同肠内营养支持时机对重型颅脑损伤术后并发感染的影响差异不大,重型颅脑损伤术后并发肺部感染多见,临床工作者应该针对性采取相应的预防措施,防止感染的发生。

Abstract:Objective To analyze the influence of different enteral nutrition on infection after operation in patients with severe brain injury. Methods From April 2009 to September 2011 in our hospital, the clinical data of 94 patients with severe brain injury were retrospectively analyzed,50 patients of early enteral nutrition (EEN) in group A,44 patients of delayed enteral nutrition (DEN) in group B, the postoperative infection rate, the site of infection and infection duration of two groups were compared and analyzed. Results In group A,the infection incidence was 32. 0% ,the infection duration was (6.8 ± 1.5) d,in group B,the infection incidence was 27, 3% ,the infection duration was (7.0 ± 1.2) d,the differences were not statistically significant between two groups (P 〉 0. 05) ,lung infection in both groups is more common. Conclusions The influence of different enteral nutrition on infection after operation in patients with severe brain injury has little difference, lung infection in both groups is common, Clinicians should be targeted to take appropriate preventive measures to prevent infection.
