高频振荡通气治疗新生儿呼吸窘迫综合征疗效对比研究

摘    要:目的评价高频振荡通气在治疗新生儿呼吸窘迫综合征的疗效。方法将NRDS患儿106例随机分为高频振荡通气组(HFOV组)和常频机械通气组(CMV组),比较两组治疗效果、通气后6h和24h患儿血气分析结果以及住院时间等指标。结果HFOV组治愈率显著高于CMV组(P〈0.05);且HFOV组患儿在通气后6h和12h,Pa02高于CMV组,OI、PaC02低于CMV组,差异均有统计学意义(P〈0.01);HFOV组患儿住院时间显著少于CMV组(P〈0.01)。结论高频振荡通气较常频机械通气更能有效改善呼吸窘迫综合征患儿肺功能情况,提高治愈率,减少患儿住院时间,值得推广应用。

Efficacy contrast of high frequency oscillatory ventilation in treatment of neonatal respiratory distress syndrome

Abstract:Objective To evaluate the efficacy of !figh frequency oscillatory ventilation in treatment of neonatal respira- tory distress syndrome. Methods Totally 106 infant patients with neonatal respiratory distress syndrome(NRDS) were randomly assigned into high frequency oscillatory ventilation (HFOV) group and conventional mechanical ventilation (CMV) group. The indexes including therapeutic effect, blood gas analysis results of 6h and 24h alter treatment and hospitalization time of each group were compared. Results Cure rate of HFOV group were significantly higher than CMV group. 6h and 24h after treatment,PaO2 of HFOV group were higher than CMV group while OI and PaCO2 were significantly lower than CMV group. The hospitalization time of HFOV group was shorter. Conclusion Compared to conventional mechanical ventilation, high frequency oscillatoE~ ventilation can improve the pulmonary function of the in- fants more effectively. With higher cure rate and shorter hospitalization time, HFOV is feasible to generalize in clinical application.
