214例急性髓细胞白血病染色体改变及预后分析

目的应用染色体核型分析技术,研究急性髓细胞白血病（AML）染色体改变与预后的关系。方法根据细胞遗传学改变,将214例初诊AML患者分为4组：伴t（15;17）改变患者为A组;伴t（8;21）/inv（16）改变患者为B组;正常核型及无其他组染色体改变为C组;伴t（9;22）或-7或复杂核型为D组,对各组患者进行预后分析。结果 214例AML患者染色体畸变率为57.01%。214例患者的平均生存时间为（13.16±8.497）月,A、B、C、D各组3年生存率分别为97.30%、86.36%、72.04%、47.5%,差异具有统计学意义（P值为0.001）。结论染色体是AML独立的预后因素,用于诊断及预后分析,有利于个体化治疗。

Analysis of chromosome and prognosis in 214 cases of acute myeloblastic leukemia

Objective To investigate the cytogenetic change and prognosis in patients with acute myeloblastic leukemia by karyotype analysis.Methods According to karyotypes characteristics,214 newly diagnosed AML were separated into 4 subgroups：patients with t（15;17）（group A）,patients with t（8;21）/ inv（16）（group B）,patients with t（9;22）/-7/complex karyotypes（group D）and others（group C）.The clinical data of patients was retrospectively analyzed in order to study the clinical significance.Results Chromosomal aberrations were detected in 57.01% from 214 cases patients of AML.The median survival time of 214 cases of patients were（13.16±8.497）months,and the probabilities of 3-year overall survival（OS）of the four groups were 97.30%,86.36%,72.04% and 47.5%,respectively,there were significantly different（P=0.001）.Conclusion Change of cytogenetic seen in AML is an independent factor of prognosis,it is helpful for the diagnosis and prognosis,and contribute to individualized treatment.
