2型糖尿病合并肥胖患者代谢状态的临床分析

摘    要:目的通过对肥胖和非肥胖的2型糖尿病患者的代谢状态各项指标的对比分析,探讨2型糖尿病合并肥胖患者代谢紊乱的临床特点。方法将300例2型糖尿病患者按体重指数(BMI)随机分为2组,肥胖组150例(BMI≥25kg/㎡)和非肥胖组150例(BMI〈25kg/㎡).测定血压(SBP/DBP)、空腹血糖(FPG)、糖化血红蛋白(HbA1C)、血总胆固醇(Tch)、甘油三酯(TG)、高密度脂蛋白胆固醇(HDL-c)、低密度脂蛋白胆固醇(LDL-c),24h尿白蛋白排泄率(UAER),血浆空腹胰岛素(F-ins)。结果 2型糖尿病肥胖组SBP,TG,LDL-c,UAER,F-ins明显高于非肥胖组,HDL-c较非肥胖组明显低,两组间比较均有统计学意义;Tch,FPG,HbA1C水平两组之间无明显差异。结论肥胖的2型糖尿病患者有明显的代谢紊乱.BMI是影响糖尿病患者脂代谢、血压、胰岛素水平及尿白蛋白排泄率的重要因素.对2型糖尿病伴有肥胖的患者应予以重视,对其代谢异常进行综合治疗。

Abstract:Objective To investigate characteristics of the metabolic state in type 2 diabetes patients with obesity for prevention and management of diabetes complication.Methods Totally 300 cases of type 2 diabetes patients were divided into two groups by BMI,in which there are 150 obese patients and 150 nonobese patients.The levels of blood pressure(SBP/DBP),fasting plasma glucose,HbAlC,plasma insulin(F-ins),blood lipid(Tch,TG,LDL-c,HDL-c).Results Systolic blood pressure(SBP)level was significantly higher in type 2 diabetes with obesity than those in control.TG and LDL –c levels significantly increased,and HDL-c level significantly decreased in obesity group.Urine albu-min excretion rate of 24 hours significantly decreased in obesity group.Conclusion Obesity type 2 diabetes patients suffer from a metabolic disorder,thereby,type 2 diabetes patients with obesity must attach great importance for effectively controlling the metabolic disorder and preventing the development of complications.
