男性尿道感染病征指标诊断淋菌性和非淋菌性尿道炎的信度及效度评价

目的　评价常见男性尿道感染病征诊断淋菌性尿道炎 (GU)、非淋菌性尿道炎 (NGU)的真实性 (效度 )及可靠性 (信度 ) ,为今后此类诊断提供依据。方法　记录患者就诊时尿道口潮红、粘脓性分泌物、稀脓性分泌物、尿道不适、尿频尿急 5项病征指标 ,同时用培养法检测淋球菌、解脲支原体 (UU)和人型支原体(MH) ,用抗原检测法检测衣原体 (CT) ,并对结果进行分析。结果　尿道不适、粘脓性分泌物对于GU的诊断具有较高敏感性 ,粘脓性分泌物信度最高。尿道口红及稀脓性分泌物诊断NGU的效度及信度均较高。结论　粘脓性分泌物及尿道口红、稀脓性分泌物和尿道口红分别为诊断GU及NGU的重要指标 ,有助于临床进行病征处理。

Validity and Reliability of Five Male Urogenital Infection Signs on the Diagnosis of Gonococcal or Nongonococcal Urethritis

Objective To determine the validity and reliability of five male urogenital infection signs on the diagnosis of gonococcal or nongonococcal urethritis. Methods Five male urogenital infection signs including redness of urethra meatus, urethroblennorrhea, urethra thin discharge, discomfort of urethra, urgency-frequency of 416 outpatients had been recorded. At the same time, gonococcus, Ureaplasma urealyticum (Uu), Mycoplasma hominis (Mh) and chlamydia trachomatis(CT) of all patients were detected. Results Discomalmfort of urethra and urethroblennorrhea were the most sensitive signs on the diagnosis of gonococcal urethritis, while urethroblennorrhea had the highest validity. Both redness of urethra meatus and urethra thin discharge all had good validity and reliability on the diagnosis of nongonococcal urethritis. Conclusions Urethroblennorrhea, redness of urethra meatus and urethra thin discharge were the most important signs on the diagnosis of gonococcal urethritis and nongonococcal urethritis respectively.
