摘    要:目的探讨磁共振成像(MRI)技术协助诊断胆红素脑病的临床价值。方法回顾性分析2012年1月-2012年12月期间,对本院14例高胆红素血症患儿行头颅MRI检查的临床资料。结果高胆红素血症14例中,有胆红素脑病早期临床表现的5例,有痉挛期临床表现的1例,MRI异常者有5例(去除可疑1例),均显示双侧苍白球T1WI信号及T2-FLAIR(抑水系列)呈对称性高信号,在T2WI呈正常信号,脑干听觉诱发电位(BAEP)异常有7例。结论 MRI能为早期高胆红素脑病提供客观指标,尤其对临床可疑病例的诊断方面有较高的临床价值。

Abstract:Objective: To evaluate the clinical application value of magnetic resonance imaging (MRI) in assisted diagnosing bilirubin encephalopathy among neonatal infants. Methods: A retrospective study of 14 neonatal infants with hyperbilirubinemia admitted to department of neonatology from Jan. 2012 to Dec. 2012 was conducted. The data of clinical manifestation and laboratory results of these infants were retrieved from medical records. Cerebral MRI and brainstem auditory evoked potentials (BAEP) were tested for evaluating the cerebral injury.The diagnostic value of cerebral MRI for early neonatal encephalopathy caused by hyperbilirubinemia was assessed. Results: Of 14 infants, 6 (42.8%) were considered as having an early clinical manifestation of neonatal bilirubin encephalopathy. The abnormal MRI signal, manifested by hyperintensities of T1WI and T2- FLAIR in the bilateral globus pallidus, were found in 5 cases of these infants. The abnormality of BAEP was founded in seven infants. Conclusion: MRI abnormal signal might be a useful and objective parameter in early assessingcerebral injury caused by hyperbilirubinemia, in particular in the cases of high suspicion.
