简易鼻塞持续气道正压呼吸支持装置在婴幼儿危重症中的应用

摘    要:〖目的〗探讨简易鼻塞持续气道正压呼吸支持(NCPAP)装置在婴幼儿危重症中的应用。〖方法〗购买空氧混合仪,加温湿化罐等配件,设计组装简易NCPAP装置,于2001年1月至2002年5月,对本院53例患肺炎、毛细支气管炎、左向右分流先天性心脏病等合并有呼吸衰竭的患儿给予NCPAP,观察其对改善血pH、二氧化碳分压(PaCO2)、氧合(PaO2/FiO2)及呼吸频率(R)、心率(P)效果,预后和相关不良反应。〖结果〗应用NCPAP后53例中40例呼吸困难吸气三凹征和气促程度在1~6h内消失,11例有改善,2例病情加重。对资料完整的20例统计,NCPAP(1~6h)后氧合显示改善(P<0.05),pH与PaCO2的变化无显著差异性(P>0.05)。〖结论〗简易NCPAP装置用于婴幼儿呼吸衰竭安全而有效。

Abstract:Objectives To explore the clinical application of simple assembling device for nose closure persistent air passage(NCPAP) respira-tion in the treatment of critically ill infants. Methods The simple assembling device for NCPAP respiration was designed andmade up of buying air-oxygen mixture instrument. The accessories of thermohygrographic pot were supplemented. Totally fifty-three infants suffering from pneamonia, bronchiolitis and left to right diffluence congenital heart disease complicated with respiratoryfailure were treated by this simple NCPAP device in our pulmonary intensive care unit(PICU) from Jan.2001 to May,2002. Theirimprovement in blood pH value, PaCO 2 , oxygenation(PaO 2/FiO 2 ),respiratory rate(R),pulse rate(P),prognosis and related side-effectswere observed. Results After using simple NCPAP device, dyspnea , three depression signs of inspiration and panting in fortyout of 53 infants disappeared within 1~6 hours .Improvement of symptoms was seen in 11 cases ,but 2 cases became worse. Havinganalyzed statistically 20 cases recorded with integral data, their oxygenation seemed better after using simple NCPAP device for 1~6hours(P<0.05). Their blood pH and PaCO 2 revealed no significant change(P>0.05). Conclusion The simple assembling device forNCPAP respiration is effective and safe for treatment of infantile respiratory failures .[
