摘    要:目的 探究早期血清Tau蛋白水平对急性缺血性脑卒中后痴呆的预测价值。方法 选择2014年12月至2015年8月安徽医科大学第二附属医院神经内科明确诊断为缺血性脑卒中的住院患者87例,在发病72 h内抽取空腹静脉血,采用ELISA法对血清Tau蛋白进行测定,并在发病3个月时采用简易智能量表(MMSE)测定患者认知功能。根据发病3个月的MMSE评分,并参考Hachinski缺血评分及DSM-IV诊断标准,将患者分为卒中后痴呆组和非痴呆组。比较两组的Tau蛋白水平,并将Tau蛋白水平与MMSE评分进行相关性分析。结果 急性缺血性脑卒中3个月后卒中后痴呆发生率为26.4%。痴呆组72 h内血清Tau蛋白水平(107.98±14.50)pg/mL,非痴呆组为(75.17±6.26)pg/mL;卒中后痴呆患者血清Tau蛋白水平明显高于卒中后非痴呆组(t=14.752,P<0.05)。偏相关分析,Tau蛋白水平与MMSE呈负相关(r=-0.144,P<0.05)。结论 早期血清Tau蛋白水平或可预测缺血性脑卒中后痴呆的发生。

Abstract:Objective To study the predictive value of serum Tau protein in post stroke dementia.Methods The study recruited 87 hospitalized patients.Tau protein was measured by ELISA in 72 hours.Intelligent level of post stroke dementia was measured by mini-mental state examination(MMSE) and was diagnosed by Hachinski Ischemic score and DSM-IV,and the patients were grouped into dementia and no dementia group.The Tau protein levels were compared in two groups,and the relationship between Tau protein levels and MMSE scores was analyzed.Results The incidence of post stroke dementia was 26.4% after 3 months.The serum levels of Tau protein was 113.95(94.33-120.13) pg/mL in dementia group and 73.45(69.98-80.45) pg/mL in no dementia group.Tau protein levels of post stroke dementia were significantly higher in patients without dementia with acute cerebral infarction(t=14.752,P<0.05).According to partial correlation analysis,the level of Tau protein was negatively correlated with MMSE(r=-0.144,P<0.05).Conclusion Serum Tau protein levels may predict cognitive function in patients with ischemic stroke.
