钛质羟基磷灰石听骨膺复物及自体听小骨在鼓室成形术中的应用

摘    要:目的 探讨不同材料在听骨链重建术的应用效果和不同术式鼓室成形术后听力效果。 方法 回顾分析了 2 3例慢性化脓性中耳炎及外伤性听骨链中断患耳 ,分别采用钛质 ,羟基磷灰石和自体听小骨作为听骨链重建材料 ,并分别采用改良鼓室乳突根治术及单纯鼓室成形术。 结果 术后近期复查纯音测听 ,总有效率 (听力提高≥ 1 0dBHL者 ) 86 .9% (2 0 /2 3) ,改良鼓室乳突根治术术后听力提高不如单纯鼓室成形术明显 ,镫骨上结构是否完整对术后听力变化无显著影响。 结论 应该根据中耳病变的不同情况 ,考虑选择何种听骨材料 ,以期达到最理想效果。鼓室成形术后听力提高效果 ,主要受炎症严重程度之影响

Abstract:Objective To investigate the hearing results of various materials in ossicular chain reconstruction, as well as those in open tympanoplasty and simple tympanoplasty. Methods 23 ears of chronic suppurative otitis media and traumatic interruption of ossicular chain were analyzed, and the operation method were modified radical tympanomastoidectomy and simple tympanoplasty. Results 20 ears had a hearing improvement of more than 10 dBHL(86.9%), the hearing effects was better in simple tympanoplasty than that of modified radical tympanomastoidectomy, whether the suprastapedial stucture was intact. Conclusion The choice of the most apropriate material depends on the actual situation of each case, so as to get the best hearing improvement. Inflamation acts as the main role in post operative heating effect.
