手术前单疗程化疗对非小细胞肺癌患者肺功能影响的临床研究

目的研究ⅢA期非小细胞肺癌(NSCLC)患者手术前单疗程化疗前后肺功能的变化,评价其对术前肺功能的影响。方法从2002年8月至2003年2月,对72例ⅢA期NSCLC进行术前化疗1个疗程。其中鳞癌38例采用CAP方案,腺癌34例采用FAD方案治疗。化疗前及化疗结束后10 d检查第一秒用力呼出量(FEV1%)、一氧化碳弥散能力(DLCO%)、动脉血气分析(PaO2、PaCO2)做自身对照。结果手术前化疗前后FEV1%、DLCO%、PaO2、PaCO2分别为78.37%±1.05%、79.06%±0.92%;80.48%±0.13%、80.25%±0.12%;(83.59±0.66)mmHg、(83.90±0.68)mmHg;(39.18±0.43)mmHg、(39.09±0.38)mmHg。两组比较差异无统计学意义(P<0.05)。结论手术前单疗程化疗对CNSCLC患者肺功能无明显影响。

Clinical observation on pulmonary function of 1 cycle preoperative chemotherapy in the treatment of non-small cell lung cancer

Objective To determine 1 cycle preoperative chemotherapy in the treatment of stage Ⅲ A NSCLC and its effects on pulmonary function. Methods Between Aug 2002 to Feb 2003, 72 patients with stage Ⅲ A NSCLC were treatment by l cycle preoperative chemotherapy in which the regimens included CAP in 38 cases of squamous carcinoma, FAD in 34 cases of adenocarcinoma. We conducted pulmonary function tests before and 10 day after chemotherapy, included FEV1%, DLCO%, PaO2 , PaCO2. Results Before preoperative chemotherapy FEV1 （ 78. 37% ±1.05% ）, DLCO （ 79.06% ±0. 92% ）, PaO2 [ （83.59 ± 0. 66 ） mmHg ], PaCO2 [ （ 39. 18 ± 0. 43 ） mmHg ], after preoperative chemotherapy FEV, （80.48% ±0. 13%）, DLCO （80.25% ±0. 12%）, PaO2[ （83.90 ±0.68） mmHg], PaCO2 [ （39.09 ± 0. 38 ） mmHg]. There are no significant differences in the extent of improvement of pulmonary function between before and after preoperative chemotherapy. Conclusion The results demonstrate that the 1 cycle preoperative chemotherapy was no significant differences of patients＇ pulmonary function with stage Ⅲ A NSCLC.
