MRCP与ERCP对阻塞性黄疸诊断价值的比较

目的：比较磁共振胰胆管造影（MRCP）和内镜下逆行胰胆管造影术（ERCP）对阻塞性黄疸的诊断价值。方法：30例阻塞性黄疸患者分别行MRCP和ERCP，其中1例行ERCP失效改行经皮肤胆管造影（PTC）。MRCP采用重T2加权及超快自旋回波水成像技术进行，ERCP按常规方法进行。结果：30例行MRCP和29例，ERCP均获成功。MRCP和ERCP总的诊断准确率分别为90％和96．7％；对恶性狭窄的诊断准确率分别为66．6％和88．9％；对胆总管结石诊断的准确率为100％。结论：MRCP为无创性检查，可作为阻塞性黄疸病因诊断的首选方法，但不能取代ERCP，两者结合可弥补各自的不足，提高对阻塞性黄疸病因诊断的准确性。

A comparative of diagnostic value of magnetic resonance cholangiopancreatography and endoscopic retrograde cholangiopancreatography in obstructive jaundice

Objective To compare the diagnostic value of magnetic resonance cholangiopancreatogra-phy(MRCP)and endoscopic retrograde cholangio pan-creatography(ERCP)in obstructive jaundice. Methods MRCP and ERCP were performed in 30 patients with obstructive jaundice. One patient underwent perctuaneoustranshepaticcholangiography (PTC) for being failed in the performance of ERCP.The technique of T2-Weighted fast spin-echo fluid image was used in MRCP. ERCP were performed in routine way. Results There were 30 cases successfully diagnosed by MRCP and 29 cases successfully diagnoses by ERCP.The over all concordance rates of MRCP and ERCP were 90% and 96.2% respectively.In malignant common duct stricture the concordance rates of MRCP and ERCP were 66.6% and 88.9% respectively.In choledo-cholithiasis the concordance rates of MRCP and ERCP were 100.0%. Conclusion The non-invasive examination,MRCP can be the frist choice as a diagnosis method for obstructive jaundice,but it can not replace ERCP.Those two methods are complementary,which can promote the rate of accuracy of diagnosis.
