摘    要:[目的]探讨本院送检标本中检出鲍曼不动杆菌的临床分布情况及耐药性,为临床合理选用抗生素提供实验室依据.[方法]回顾性分析2008 年1 月至2010 年12 月本院分离的109鲍曼不动杆菌的临床分布、感染特点及其药敏结果,药敏试验采用最低抑菌浓度(MIC)法.[结果]109 株鲍曼不动杆菌,标本主要来源于痰液68.8%(75/109),科室主要分布于ICU 28.4%(31/109),其次为呼吸内科24.8%(27/109)和神经内科21.1% (23/109);鲍曼不动杆菌对头孢哌酮/舒巴坦(2.3%)耐药性最低,其次是亚胺培南(8.6%)、美罗培南(12.3%)和阿米卡星(9.7% );对其他抗生素均有不同程度的高耐药性、多重耐药性.[结论]鲍曼不动杆菌对多种抗菌药物耐药性高且耐药率增加较快,临床应加强监测,合理使用抗菌药物.

Abstract:[Objective] To analyze the clinical distribution and drug resistance of Acinetobacter baumannii in order to provide references for rational use of antibiotics in the clinic. [Methods] The characteristics of clinical infection and results of drug susceptibility of 109 strains of Acinetobacter baumannii from January 2008 to December 2010 in our hospital were analyzed retrospectively. The susceptibility was tested by minimum inhibitory concentration(MIC) method. [Results] Totally 109 strains of Acinetobacter baumanii isolated were mainly from sputum samples(68.8 %). All of them were mainly distributed in intensive care unit (28.4 % ) 31/109), followed by respiratory medicine department ( 24.8 % ) and neurology department(21.1 %). Acinetobacter baumannii was least resistant to cefoperazone/sulbactam(2.3 % ), followed by imipenem(8.6 % ), meropenem(12.3%) and amikacin(9.7 %). It had higher degree of drug resistance rates to the other antibacterials and multidrug resistance. [Conclusion] The drug resistance of Acinetobacter baumannii is changing continu- ously, and the resistance to various of antibacterials is high, and the resistance rate increases rapidly. It is necessary to strengthen the clinic monitoring and the rational use of antimicrobial drugs.
