原发性高血压及相关危险因素与脉搏波传导速度的相关性

摘    要:[目的]探讨原发性高血压(EH)及相关危险因素与脉搏波传导速度(pulse wave velocity,PWV)的相关性.[方法]收集2009年6月至2010年12月在湖南省人民医院诊治且病历资料较完整的160例EH患者,按血压水平由高至低分为三组:高血压1级50例,高血压2级52例,高血压3级58例.另选择门诊健康体检者50名作为对照组.收集研究对象的一般情况,血生化指标、PWV及踝/臂血压指数(ankle –brachial index,ABI)的结果.[结果]高血压各组的PWV增大及ABI异常的发生率明显高于对照组,并随血压等级增加而增加,差异有统计学意义(P〈0.05).Logistic回归分析显示,增龄、男性、吸烟、高胆固醇血症及低密度脂蛋白胆固醇是PWV增大的独立危险因素(P〈0.05),而脂蛋白(a)、尿酸、肌酐与PWV无明显相关性(P〉0.05).[结论]原发性高血压患者PWV增大,其程度和ABI异常的发生率均随血压分级增高而增加;增龄、男性、吸烟、高胆固醇血症及低密度脂蛋白胆固醇是PWV增大的独立危险因素.

Abstract:[Objective] To explore the relationship of essential hypertension(EH) and its related risk fac tors with pulse wave velocity(PWV). [Methods] Intact clinieal data of 160 EH patients admitted to Hunan provineial people's hospital from June 2009 to Dec. 2010 were collected. According the level of blood pressure, all patients were divided into hypertension grade I group( n ~ 50), hypertension grade Ⅱ group( n = 52) and hypertension grade Ⅲ group( n = 58). In addition, 50 healthy subjects were selected as control group. Gener-al condition, biochemical indicators, PWV and ankle-branchial index(ABI) of all subjects were collected. [Re-sults] Incidence of inereased PWV and normal ABI in hypertension groups were obviously higher than those in control group, and increased with the grade of blood pressure, and there were significant differences( P〈0.05). Logistic regression analysis showed that aging, male, smoking, hypercholesterolemia and low density lipoprotein-cholesterol(LDL-C) were the main risk factors of increased PWV. Lipoprotein(a), uric acid and creatinine had no correlation with PWV( P 〉0. 05). [Conclusion] Hypertension may increase PWV. The de-gree of increased PWV and the incidence of normal ABI increase with the grade of blood pressure. Aging, male, smoking, hypercholesterolemia and LDL-C are the main risk factors of increased PWV.
