应用阿立哌唑处理抗精神病药引起催乳素血症的初步研究

目的]探讨服用阿立哌唑处理抗精神病药引起的高催乳素血症的效果.方法]对42例服用抗精神病药处于精神症状稳定期（PANSS＜60分）,但出现药源性高催乳素血症的女性精神分裂症患者,在4周内完全替换成阿立哌唑,并继续服用该药12周.临床评估包括：分别在基线、研究结束时,采用阳性与阴性症状量表（PANSS）及临床疗效总评量表（CGI-I）评定疗效、检测血清催乳素水平（PRL）;应用副反应量表（TESS）在替换前及替换后1、2、4、6、8、12周评定药物不良反应.结果]用阿立哌唑替代抗精神病药16周后,PANSS总分、各因子分及CGI-I分与替换前差异均无统计学意义（P〉0.05）,而血清PRL水平与替换前有明显降低,不良反应患者均可耐受.结论]对于抗精神病药引起的药源性催乳素血症,应用阿立哌唑来替代,是处理此副作用的一个有效对策.

Primary Study of the Efficacy of Aripiprazole for the Treatment of Hyperprolactinemia Caused by Antipsychotics

Objective]To explore the efficacy of aripiprazole for the treatment of hyperprolactinemia caused by antipsychotics. Methods] Forty two female patients with schizophrenia who were psychiatrically stable when taking antipsychotics（PANSS〈60）, but had drug-induced hyperprolactinemia turned to take aripiprazole within 4 weeks and continued for 12 weeks. Clinical assessments included efficacy evaluation by using positive and negative symptom scale（PANSS） and clinical global impression scale（CGI-I）, the detection of serum prolactin（PRL） and drug adverse reaction assessment by using treatment emergent symptom scale（TESS） before drug replacement and 1, 2, 4, 6, 8 and 12 weeks after drug replacement. Results] After aripiprazole for replacing antipsychotics for 16 weeks, there was no significant difference in total PANSS scores, each factor scores and CGI-I scores between before and after replacement（ P 〉0.05）. Compared with before replacement, serum PRL levels obviously decreased after replacement. Patients could tolerate all the adverse reaction. Conclusion] Aripiprazole for replacing antipsychotics for the treatment of hyperprolactinemia induced by antipsychotics is an effective method.
