不同超前镇痛时点应用氯诺昔康对术后镇痛效果的观察

摘    要:目的 观察与比较不同超前镇痛时点应用氯诺昔康(Lornixicam)对术后疼痛治疗的效果,并评估其对尿路内镜术后疼痛的治疗作用.方法 择期进行URL或MPCNL患者各150例,分别把MPCNL和URL术式患者分为M组和U组,每个组中均有两个观察组(M1和M2,U1和U2)和一个对照组(M3,U3),每组均为50例,M1和U1组于手术开始前10 min静注氯诺昔康(奈科明有限公司)0.15 mg/kg,M2和U2组于手术结束前10 min静注氯诺昔康0.15 mg/kg,M3和U3组不使用任何镇痛类药.结果 M1组、M2组和M3组Vmax和V24分别与U1组、U2组和U3组相对应的Vmax和V24比较均有显著差异(P<0.01).结论 氯诺昔康在手术结束前应用的镇痛治疗结果优于手术开始应用,对URL术后疼痛的治疗效果优于MPCNL.

Abstract:Objective To compare Pre-emptive analgesia with Lornixicam on different time for post-operative pain,and to evaluate the therapeutic effects of the postoperative analgesia after the urologic endoscopic operation.Methods 150 cases who were received minimally invasive percutaneous nephrolithotomy and 150 cases with ureteroscopic lithotripsy,and all cases were divided into 6 groups(M_1,M_2,M_3,U_1,U_2 and U_3),50 cases in each group.Group M_1 and U_1 was injected with Lornixicam(0.15 mg/kg) 10 min pre-operation.Group M_2 and U_2 was injected with Lornixican(0.15 mg/kg) before the end of the operation.Group M_3 and U_3 had no medicine.Results The maximum degree and the degree of pain on 24 hours after operation in Group M_1,M_2,and M_3 show significant different to U_1,U_2,U_3 group(P<0.01).Conclusion The effect of pre-emptive analgesia with Lornixicam used better before the end of operation than the operation,and its therapeutic effect was better for ureteroscopic lithotripsy than minimally invasive percutaneous nephrolithotomy.
