核仁形成区对非何杰金淋巴瘤与慢性淋巴结炎鉴别诊断价值的研究

对核仁形成区计数及直径在鉴别非何杰金淋巴瘤(89例)和慢性非特异性淋巴结炎(51例)的诊断价值进行研究.结果:核仁形成区计数的敏感性为92.1%,特异性为92.2%,准确性为92.1%.核仁形成区计数均值在非何杰金淋巴瘤与慢性非特异性淋巴结炎的差别有显著性意义(P=0.000).在40例形态不典型病例中,病理诊断及临床诊断的特异性过低(分别为61.5%和0%),严重影响准确性,而核仁形成区计数仍有较高的诊断价值(敏感性92.6%、特异性84.6%、准确性90%).

A STUDY ON THE DIAGNOSTIC VALUE OF ARGYROPHIL NUCLEAR ORGNIZE REGIONS IN DISCRIMINATING NON-HODKINGS LYMPHOMA FROM CHRONIC NONSPECIFIC LYMPHADENITIS

The diagnostic value of argyrophil nuclear orgnize reions(AgNOR) in discriminating Non -Hodking's lymphoma (NHL) from chronic nonspecific lymphadenitis (CNL) were studied.When a series of patients consisting 89 NHL and 51 CNL were classed by means of AgNOR count, the diagnostic sensitivity, specificity and accuracy were 91.l%(.82/89), 92.9% (47/51) and 92.1% (129/140)respectively.There were 40 patients in which accurate diagnosis were difficult by ordinary pathologic methods and clinic examination, but most of these patients were correctly classed by means of AgNOR count technic in which the sensitivity, specificity and accuracy reached 92.6% (25/27), 84.6% (11/ 13) and 90% (36/40) respectively.When there are difficulties in diagnosis by ordinary pathologic method, we propose uoing a combined diagnostic method which includis the AgNOR count, preliminary clinical diagnosis and AgNOR diameter.A good diagnostic speificity (92.3%, 12/13) was achieved in the above mentioned 40 diffcult cases byuoing this method.
