产超广谱β-内酰胺酶肺炎克雷伯氏菌的耐药性监测

目的 对肺炎克雷伯氏菌进行超广谱β-内酰胺酶检测及耐药性测定。为产ESBLs细菌的监控和治疗提供依据。方法 对临床分离的肺炎克雷伯氏菌采用K—B纸片扩散法．严格按照美国NCCLS制定的标准，进行ESBLs检测并对19种抗生素的耐药性测定。结果 在41株肺炎克雷伯氏菌中，检出16株产ESBLs阳性率为39．0％。ESBLs阳性株除对亚胺培南、头孢哌酮／舒巴坦高度敏感外，对其它抗生素的耐药性明显高于ESBLs阴性株。结论 产ESBLs的肺炎克雷伯氏菌在临床分离株中阳性率较高．且对多种抗生素耐药．应加强对ESBLs的检测和其感染者的治疗。

Survey on antibiotic-resistance of extended-spectrum β-lactamases producing Klebsiella pneumoniae

Objective To survey ESBLs and drug resistance of Klebsiella pneumoniae and guide the control and treatment of ESBL producing bacteria. Methods Forty-one strains of Klebsiella pneumoniae were isolated from clinical specimens. ESBL-producing strains were confirmed by the NCCLS phenotypic confirmatory test, the antibacterial susceptibility testing on 19 kinds of antibiotics was performed by K-B method. Results Among 41 strains of Klebsiella pneumoniae , ESBL producing bacteria accounted for 16 (39.0%). ESBL producing strains were susceptible to imipenem and cefoprazone/sulbactam, showing more resistant to other antibiotics than ESBLs negative strains. Conclusion The incidence of ESBLs in Klebsiella pneumoniae was rather high, and these strains were usually resistant to conventional antibiotics.
