摘    要:目的:探讨纤维蛋白原(FIB)和血小板平均体积(MPV)与急性脑梗死(ACI)的关系。方法:分析429例ACI患者(ACI组)和429例非脑梗死患者(对照组)的血小板和凝血系统相关参数水平,并根据ACI组的美国国立卫生研究院卒中量表(NIHSS)评分将ACI组分为轻、中、重度亚组。结果:与对照组比较,ACI组的PLT水平显著降低(P<0.01),MPV、血小板分布宽度(PDW)及纤维蛋白原(FIB)水平显著升高(P<0.01)。多因素Logistic回归分析显示,MPV和FIB与脑梗死呈独立相关(OR=1.628,95%CI为1.427~1.857;OR=7.115,95%CI为4.981~10.164);FIB与MPV水平两者相互间无明显相关性。ACI组各亚组FIB水平两两比较差异有统计学意义(P<0.01),ACI患者FIB水平与NIHSS评分呈正相关(r=0.451,P<0.01)。结论:FIB、MPV可能为引起急性脑梗死的重要危险因素,且FIB与病情的严重程度呈显著正相关。

Abstract:Objective: To investigate the relationship of fibrinogen (FIB), mean platelet volume (MPV) and acute cerebral infarction (ACI). Methods: The level ofplatelets system platelets (PLT), MPV, plateletcrit, platelet distri- bution width (PDW) and the coagulation system (prothrombin time, international normalized ratio, FIB, activated partial thromboplastin time) of 429 ACI cases (ACI group) and 429 cases of Non-cerebral infarction group (con- trol group) were analyzed. According to the National Institutes of Health stroke Scale (NIHSS) in ACI pa- tients, the ACI group was divided into mild, moderate and severe subgroups. Results: Compared with the con- trol group, the PLT levels were significantly lower (P〈0.01) whereas the MPV, PDW and FIB levels were signif- icantly higher (P〈0.01) in ACI group. Logistic regression analysis showed that MPV and FIB were independently associated with ACI (OR = 1.628, 95% C1:1.427-1 .857; OR = 7.115,95% CI :4.981-10.164). The FIB level showed no significant correlation with MPV level. The FIB levels between the subgroups of ACl group were sig- nificantly different (P〈0.01). In ACI patients, the FIB levels and NIHSS score were positively correlated (r = 0.451, P〈0.01). Conclusion: FIB and MPV levels may be the important risk factors for ACI.
