| 摘 要： | [目的]通过监测外科手术、创伤病人入ICU时的降钙素原(PCT)水平,评估PCT对外科手术、创伤病人院内感染的预测作用。[方法]纳入年龄大于18岁、创伤或手术后需要进入ICU监护的病人73例。监测PCT水平及院内感染情况。[结果]PCT≥0.05ng/ml预测外科术后、创伤病人院内感染的敏感度为100%,特异度37.5%,准确度71.2%,阳性预测值32.7%,阴性预测值100%,阳性似然比1.60,阴性似然比为0。PCT≥0.25ng/ml预测外科术后、创伤病人院内感染的敏感度为37.5%,特异度97.0%,准确度64.4%,阳性预测值93.8%,阴性预测值56.1%,阳性似然比12.5,阴性似然比为0.64。入ICU的PCT值预测外科术后、创伤病人院内感染的ROC曲线下面积为0.893。[结论]对创伤及外科手术后病人进行PCT监测,有助于早期发现院内感染高危病人,可指导临床抗生素治疗。
