血液透析患者股静脉留置导管相关性菌血症菌群分布及药敏分析

摘    要:目的了解我院血液透析患者股静脉留置导管相关性菌血症常见细菌分布和对抗生素耐药情况。方法对33例股静脉留置导管相关性菌血症患者血培养细菌及药敏情况进行回顾性分析。结果 33例标本中分离出17种病原菌,葡萄球菌属15例(45.5%),其他革兰阳性球菌7例(21.2%),革兰阴性菌6例(18.2%),真菌5例(15.2%)。检出的葡萄球菌中9株MRS阳性,革兰阴性菌中1株ESBL阳性。葡萄菌属、肠球菌等革兰阳性菌对万古霉素、喹奴普汀/达福普汀敏感,革兰阴性菌对亚胺培南、美罗培南敏感。结论我院血液透析患者股静脉留置导管相关性菌血症菌群以革兰阳性菌为主,经验性治疗可选用万古霉素,同时警惕革兰阴性菌的院内感染和真菌感染。

Abstract:Objective Understanding of our blood dialysis patients strands of central-vein indwelling catheter bacteremia common bacteria distribution and drug resistance to antibiotics.Methods To 33 cases of shares to central-vein indwelling catheter bacteremia patient blood culture bacteria and drug susceptibility conditions were analyzed retrospectively.Results In 33 cases were isolated specimens of 17 kinds of pathogens,staphylococcus 15 cases(45.5%),other gram-positive cocci 7 cases(21.2%),gram-negative bacterium in 6 cases(5),18.2% fungi(15.2%).Detection of staphylococcus aureus,MRS strains of 9 for in the gram-negative bacterium ESBL strains.Grape coccus of intestinal bacteria,such gram-positive bacterium to vancomycin,leptin/slave leptin sensitive and incumbency of gram-negative bacterium,e.faecalis sensitive.Conclusion Our hemodialysis patients strands of central-vein indwelling catheter bacteremia bacterium to gram-positive bacterium empiric therapy,optional also alert vancomycin,gram-negative bacterium of nosocomial infection and fungal infection.
