超声造影在肝脏局灶性病变诊断中的价值

[目的]通过观察超声造影（contrast—enhanced ultrasound，CEUS）在肝脏局灶性病灶中各时相的回声变化规律，对照增强CT各病灶的特点，探讨CEUS对肝脏局灶性病变的诊断价值。[方法]35例常规超声检查不能完全明确诊断的肝脏局灶性病变患者，采用造影剂SonoVue以及百胜公司Technos DU 8进行超声造影研究。[结果]14例原发肝癌中10例呈典型表现，为“快进快出”。6例肝转移癌表现多样，动脉期可呈环形或整体增强，肝恶性肿瘤的消退时间早于其他肝脏局灶性病变（P〈0．05），门脉期或延迟期表现为相对低回声。6例肝血管瘸有其特有征象动脉期开始缓慢向心性结节环形增，持续数分钟后才消退。与增强CT检查所观察的各病变的增强变化规律较一致，两者无显著性差异。[结论]超声造影技术可明显提高对肝脏局灶性病变的诊断价值。

Value of contrast-enhanced ultrasound in diagnosis of focal liver lesions

[Objective ] To investigate the rule of the lesions in real time contrast-enhanced ultrasound,compared to enhanced CT,in the diagnostic value of liver neoplasm.[Methods] Thirty-one patients with unconfirmed liver neoplasm were studied.[ Results ]Among the 14 cases of HCC,10 were typical,presented with early artery enhancement and immediate wash-out.The enhancement pattern of 6 liver metastasis was diverse,including peripheral or entire degree enhancement in the arterial or portal phase with slow or fast wash-out.The 6 cases of hemangioma were enhanced centripetally in portal phase and washed out after several minutes and the other 2 nodule cases with entire enhancement.The ranges of some liver neoplasms after contrast-enhanced were different from common ultrasound and enhanced CT.[Conclusion] The contrast-enhanced ultrasound technique is sensitive and effective in diagnosing liver neoplasms.
