高血压病内皮依赖性血管舒张及分泌功能的变化

目的：探讨内皮依赖性血管舒张功能和血浆内皮素、血管性假血友病因子、纤溶酶原激活物抑制物浓度在轻、中度高血压病中的变化及其相互关系。方法：采用高分辨率超声技术，对２１例轻、中度高血压病患者与２１名正常对照者的内皮依赖性血管舒张功能进行检测，同时测定血浆ＥＴ－１、ｖＷＦ、ＰＡＩ水平。结果：高血压病组反应性充血性肱动脉舒张率较对照组明显减弱（Ｐ〈０．００１），而二组对硝酸甘油的反应无显著性差异（Ｐ〉０．０５）。在高血压病组血浆ｖＷＦ和ＥＴ－１水平明显高于正常对照组（Ｐ〈０．０５）。而二组间ＰＡＩ活性无显著性差异。结论：高血压病患者存在内皮依赖性血管舒张功能障碍，并有血浆ＥＴ－１、ｖＷＦ水平升高，将高分辨率Ｂ超技术检测内皮依赖性血管舒张功能和血浆ＥＴ－１、ｖＷＦ、ＡＰＩ水平结合起来分析可能有利于对高血压病患者病情的判断

Endothelium-Dependent Vasodilatation and Endothelial Secretion Function in Patients with Essential Hypertension

Aim:To study endothelium dependent vasodilatation, plasma levels of endothlin(ET 1),von willber factor(vWF) and plasminogen activator inhibitor (PAI) activity as well as the inter relationships between them. Methods:The endothelial dependent vasodilation was measured by high resolution B mode ultrasonograhy. Plasma level of ET 1 detected by radio immunoassay,and vWF and PAI by enzyme linked immunosorbent assay. Results: Endothelium dependent vasodilatation was much reduced in patients with essential hypertension compared with that of the control.( P <0 001). Plasma levels of ET 1, vWF in patients with essential hypertension was significantly higher than those of the control group. No significant difference in PAI activity between two groups was found. Conclusion:Our findings support that endothelium dependent vasodilatation is impaired and plasma ET 1 and vWF are increased in patients with essential hypertension.
