一次性透明鞘辅助神经内镜锁孔入路治疗高血压脑出血

摘    要:目的研究评价一次性透明鞘辅助神经内镜微创锁孔治疗高血压脑出血的临床疗效及安全性。方法回顾性分析35例采用一次性透明鞘辅助神经内镜治疗高血压脑出血患者的临床资料。结果 35例患者手术切口长度为3.0~4.0 cm,骨窗直径2.0~2.5 cm,平均血肿清除率90.21%,手术平均时间76.2 min,术中平均失血量100 ml。术后6个月GOS评分:恢复良好10例,轻度残疾10例,重度残疾9例,植物生存6例,死亡0例。结论一次性透明鞘辅助神经内镜锁孔入路治疗高血压脑出血疗效确切,其微创、精准、高效、预后良好的优点使该技术具有较高的临床应用价值。

Neuroendoscopic surgery via keyhole for hypertensive cerebral hemorrhage with disposable transparent sheaths
Abstract:Objective To evaluate the clinical effects and safety of the neuroendoscopic surgery via keyhole for hypertensive cerebral hemorrhage with disposable transparent sheaths.Methods The clinical data of 35 patients of hypertensive cerebral hemorrhage by neuroendoscopic surgery via keyhole with disposable transparent sheaths were analyzed retrospectively.Results The length of scalp incision was 3.0 to 4.0 cm, the diameter of bone window was 2.0 to 2.5 cm, the mean hematoma evacuation rate was 90.2%, the mean operation time was 76.20 min, the mean intraoperative blood loss was 100 ml.The glasgow outcome scale ( GOS) score 6 months after the operation revealed: recovered well in 10 patients, mild disability in 10, severe disability in 10, vegetative state in 6 and died in 0.Conclusions Neuroendoscopic surgery via keyhole for hypertensive cerebral hemorrhage with disposable transparent sheaths is a new form of intracranial hematoma surgery, with the advantages of less trauma, short operative time, few complications, quick recovery , and can greatly reduce mortality and morbidity, and has a good effect of neuro recovery.It is worthy of clinical application widely.
