复合手术室在脑动脉瘤夹闭术中的运用

目的探讨术中造影术在颅内动脉瘤夹闭术中的作用。方法回顾性分析45例采用复合手术方式治疗的颅内动脉瘤患者的临床资料。患者均使用了术中脑血管造影技术(iDSA)辅助进行动脉瘤瘤颈夹闭。结果所有患者的动脉瘤均得到妥善处理。有3例患者因iDSA发现瘤颈残留而重新夹闭;1例患者因iDSA发现分支血管误夹而重新夹闭;1例患者因术中未能完全夹闭瘤颈而二期行介入栓塞术。结论 iDSA能够准确判断瘤颈是否完全夹闭,以及是否误夹分支血管。复合手术的意义是为了更好地进行脑血管手术;对于脑动脉瘤是一项安全可行的,以及疗效确切的治疗策略。

Applications of hybrid operation room in cerebral aneurysm surgery

Objective To explore the use of intraoperative digital substraction angiography ( iDSA) in cerebral aneurysm surgery .Methods The clinical data of 45 patients underwent iDSA in our hybrid operating room were analyzed retrospectively . All patients were treated with intraoperative angiography ( iDSA ) technique assisted clipping . Result All aneurysms were properly treated .3 cases have been clipped again because the discovery of neck residues by iDSA , 1 case had been clipped again because the discovery of error clamping branch vessels .1 case had accepted embolization because the failure to completely clip the aneurysm . Conclusion Intraoperative DSA can accurately judge whether clipping aneurysm neck completely , and whether error clamping branch vessels .Hybrid operation is a safe and feasible ,as well as effective treatment strategy .
