阻塞性睡眠呼吸暂停低通气综合征患者血清瘦素水平及与胰岛素抵抗的相关性研究

目的探讨阻塞性睡眠呼吸暂停低通气综合征(OSAHS)患者血清瘦素水平及与胰岛素抵抗的相关性.方法选择OSAHS患者和健康者(对照组)各50例,分别行多导睡眠图检查,并测定空腹血糖、瘦素和胰岛素水平.结果 (1)OSAHS患者组的血清瘦素、空腹胰岛素水平及胰岛素抵抗指数(HOMA-IR)与对照组间差别均有显著性意义(P＜0.01).(2)OSAHS患者组血清瘦素水平、空腹胰岛素水平、HOMA-IR均与呼吸暂停低通气指数(AHI)、呼吸障碍事件最长时间、微觉醒指数呈正相关,与最低血氧饱和度呈负相关.(3)OSAHS患者血清瘦素水平与空腹胰岛素水平、HOMA-IR呈正相关.结论 (1)OSAHS患者的空腹血清瘦素水平升高,且与OSAHS病情严重程度有关.(2)OSAHS独立于肥胖等混淆因素与胰岛素抵抗存在相关性.(3)OSAHS患者血清瘦素水平与胰岛素抵抗存在相关性.

The Level of Serum Leptin and the Correlation with Insulin Resistance in OSAHS Patients

Objective To investigate the changes of fasting serum leptin and insulin level in obstructive sleep apnea-hypopnea syndrome (OSAHS) patients and to study the correlation between serum leptin level and insulin resistance in OSAHS patients.Methods Polysomnography was examined during sleep,and fasting plasma glucose,serum leptin level and insulin level were determined in 50 OSAHS patients and 50 age-matched normal controls.Results (1)Compared with control subjects,serum leptin level,fasting insulin level and HOMA-IR were significantly higher in OSAHS patients.(2)In OSAHS patients,serum leptin level,fasting insulin level and HOMA-IR were positively correlated with AHI,the longest apnea/hypopnea duration and microarousal index,and negatively correlated with the minimum oxygen saturation.(3)In OSAHS patients,serum leptin level was positively correlated with fasting insulin level and HOMA-IR.Conclusion (1)Serum leptin level was significantly higher in OSAHS patients,and serum leptin level was correlated with the severity of OSAHS.(2)OSAHS might correlate with insulin resistance of BMI independently.(3)Serum leptin level was correlated with insulin resistance in OSAHS patients.
