视黄醇结合蛋白在糖尿病早期肾病诊断中的效果评价

摘    要:目的探讨视黄醇结合蛋白(RBP)在糖尿病早期肾病诊断中的意义。方法选择101例2型糖尿病住院患者为研究对象,建立模型0(仅包括基本资料年龄、性别、BMI、血脂等),模型1(基本资料+常规肾功能指标BUN、Cr、UA)和模型2(基本资料+RBP)等3个模型,采用Logistic回归及受试者工作特征曲线(ROC曲线),基于曲线下(AUC)面积评价RBP在诊断是否患有糖尿病早期肾病中的效果。结果模型0,模型1和模型2等3个模型的AUC面积的差异具有统计学意义(AUC0=73.99%,AUC1=76.09%,AUC2=87.39%,P=0.0077)。模型2的AUC明显高于模型1及模型0(P2vs1=0.0058,P2vs0=0.0019):而模型1和模型0的AUC差异无统计学意义(P=0.33)。结论 RBP在诊断糖尿病早期肾病方面较之BUN、Cr等传统肾功能指标相比,具有更好的诊断效果。

Evaluation of the effect of retinol-binding protein on the diagnosis of early diabetic nephropathy

Abstract:Objective To investigate the effect of retinol-binding protein(RBP) on the diagnosis of early stage diabetic nephropathy.Methods All 101 cases of type 2 diabetes admitted in our hospital in 2012 were included in this research.Model 0(including baseline information: age,sex,BMI and serum lipid level),model 1(including baseline information plus common renal function index: BUN,Cr and UA) and model 2(including baseline information plus RBP) were set up.Logistic regression and the area under curve(AUC) statistic of receiver operating characteristic(ROC) curve was used to detect the application effect of RBP on the diagnosis of early stage diabetic nephropathy.Results The AUCs of model 0,model 1 and model 2 were different with statistical significance(AUC0=73.99%,AUC1=76.09%,AUC2=87.39%,P=0.0077).The AUC2 was statistically higher than AUC1 and AUC0(P2vs1=0.0058,P2vs0=0.0019).While there was no significant difference between AUC1 and AUC0(P=0.3300).Conclusion Compared with common renal function indicators,RBP is a better biomarker of early stage diabetic nephropathy.
