后腹腔镜切开取石和mPCNL治疗输尿管上段结石的对比研究

目的比较后腹腔镜输尿管切开取石和经皮微穿刺碎石术(mPCNL)治疗输尿管上段结石的疗效。方法 43例输尿管上段结石的患者接受治疗,其中腹腔镜组20例,mPCNL组23例。结果术后1周复查尿路平片,后腹腔镜组结石清除率100%,mPCNL组91.6%。结论 2种方法治疗输尿管上段结石均能取得满意疗效,当结石位于L3以上时mPCNL疗效较好,而后腹腔镜切开取石治疗L3～L5上缘的结石则可能效果更好。

Comparison of two mini-invasive surgical procedures for upper ureteral calculi

Objective To compare the effects of minimal invasive percutaneous nephrolithotomy(mPCNL) and retroperitoneal laparoscopic ureterolithotomy in management of upper ureteral calculi.Methods Forty-three patients with upper ureteral stone were enrolled in the study.Twenty cases were treated by netroperitoneal laparoscopic ureterolithotomy and 23 by mPCNL.Results In mPCNL group,the stone-free rate was 91.6% one week after operation;in retroperitoneal laparoscopic ureterolithotomy,that was 100%.Conclusion High stone-free rate can be obtained by two methods.mPCNL could be an effective surgical procedure for treatment of upper ureteral calculi(>L3);retroperitoneal laparoscopic ureterolithotomy is a selective method of minimal trauma for the treatment of ureteral calculi(L3-L5),or bigger stones.
