脑白质疏松症经颅多普勒超声及磁共振血管成像的对比研究

摘    要:目的 探讨脑白质疏松症(LA)颅内血管成像及血流动力学变化特点.方法 对207例LA患者(老年组与非老年组)和200例对照组进行经颅多普勒超声(TCD)检测,其中有78例LA患者行磁共振血管成像(MRA)检查,并加以对比分析.结果 LA组颅内各组血管平均血流速度均较对照组降低(P<0.05),老年LA组颅内各动脉Vm与非老年LA组同名血管比较血流速亦普遍减慢(P<0.05).78例LA患者行MRA检查,异常检出率为93.59%,两项检查方法对比分析无统计学意义(P>0.05).结论 增龄、低灌注和颈内动脉系统血管病变与LA形成有关.TCD及MRA检查可显示LA患者的血流动力学改变和脑血管损害,为判断病情及预后,为早期干预治疗提供依据.

Abstract:【Objective】To investigate the characteristics of MRA and hemodynamics in patients with leukoaraiosis.【Methods】207 patients with leukoaraiosis(aged group and young group) and 200 controls were examined with TCD.78 patients were examined with MRA.Then chi-square test was adopted to contrast MRA and TCD results of 78 patients.【Results】The mean velocity of LA cases was slower than that of the controls(P <0.05).The mean velocity of aged LA group was slower than that of the young LA group(P <0.05).The abnormality rate detected by MRA is 93.59%.No significant correlation existed between MRA and TCD results of 78 patients.【Conclusion】Older age,lower perfusion and change in interal carotid artery system are related to LA.TCD and MRA are helpful to understand the changes of hemodynamics and the occlusion or stenosis of intracranial artery in patients with leukoaraosis,and furthermore to know the formation and the blood supply of LA.These results can be useful in estimating the prognosis of patents and treating early.
