不同质子泵抑制剂对氯吡格雷抗血小板效应影响的研究

目的为进一步研究和探讨治疗剂量下不同质子泵抑制剂泮托拉唑、奥美拉唑对氯吡格雷抗小板活性的影响。方法选择2009年12月至2011年1月入住内蒙古自治区人民医院心血管内科的急性冠状动脉综合征(ACS)患者与择期行经皮冠状动脉介入(PCI)治疗患者112例,记录基本情况,并将他们随机分成3组:奥美拉唑+氯吡格雷组,泮托拉唑+氯吡格雷组,单用氯吡格雷组(对照组),通过流式细胞仪分别测定服药前和服药后第7天血小板P选择素阳性率,并对各项指标进行统计学分析。结果 3组间样本匹配,各组内用药前后血小板P选择素阳性率差异有统计学意义(P<0.01)。3组间治疗前比较血小板P选择素阳性率差异无统计学意义(P=0.57),治疗后比较差异仍无统计学意义(P=0.82)。结论①治疗剂量的泮托拉唑与奥美拉唑短期内对氯吡格雷的抗血小板疗效差异无统计学意义。②泮托拉唑与奥美拉唑对影响氯吡格雷抗小板效应方面差异无统计学意义。

The research of different proton pump inhibitors on antiplatelet effect influence of clopidogrel

Objective To study the influence of different proton pump inhibitors,Pantoprazole and Omeprazole,on antiplatelet aggregation to clopidogrel.Methods To choose 112 cases of patients,which were in Inner Mongoliaautonomous region people’s hospital of Cardiovascular physician form December 2009 to January 2011,were their general information recorded in detail.All patients were randomly divided into three groups.The group 1(42 cases) were given Clopidogrel and Pantoprazole;The group 2(40 cases) take Clopidogrel and Omeprazole;control group(30 cases) take only Clopidogrel.Every patient had been on Aspirin.Using the flow cytometric to analyze platelet P-selectin positive rate(%),before and seven days after taking medicine.then statistical analysis were perforned.Results The general information is matched.Comparing to the before treatment,in seventh day,platelet P-selectin positive rate(%) have significanc difference(P<0.01) in three groups.But before treatment,three groups have no significanc difference(P=0.57),and it still have no significanc difference in seventh day(P=0.82).Conclusion ①Therapeutic doses of Pantoprazole and Omeprazole has no significant effect on antiplatelet curative effect of Clopidogrel in short period.②there is no significance influence to Clopidogrel between Pantoprazole and Omeprazole
